Patient: so hi Braden how's it going <6:20:38 PM> Pooja Kamath - Patient: it's pretty good um what did you <6:20:46 PM> Braden - Patient: here <6:20:46 PM> Pooja Kamath - Patient: so what I've been what have you been up to recently what keeps you busy <6:20:49 PM> Braden - Patient: oh you know I've been working a lot and <6:20:52 PM> Pooja Kamath - Patient: mm-hmm <6:20:55 PM> Braden - Patient: I get all the bills paid take care of the house and the family <6:20:58 PM> Pooja Kamath - Patient: mm-hmm yeah very <6:21:00 PM> Braden - Patient: all the <6:21:03 PM> Pooja Kamath - Patient: how's the family <6:21:06 PM> Braden - Patient: they're good yeah it's changed much there we're all <6:21:06 PM> Pooja Kamath - Patient: yeah <6:21:08 PM> Pooja Kamath - Patient: mm-hmm well what questions do you have for this visit <6:21:10 PM> Braden - Patient: doing our thing <6:21:15 PM> Braden - Patient: I don't have questions I just need to get these darn medications failed because I think I got like five or six that might need to be grateful I honestly don't know I went to the pharmacist the other day and they were saying stuff and I don't know and they said I need to come talk to you so I'm here to talk to you <6:21:16 PM> Pooja Kamath - Patient: mm-hmm <6:21:24 PM> Pooja Kamath - Patient: okay yeah so we do have a couple of medication refills and we all so you know the kind of we get like little be like a green a yellow and a red based on kind of resonant looks like you haven't been refilling it as much as like if <6:21:55 PM> Pooja Kamath - Patient: like just haven't been refilling it as much as kind of the estimated values so what's been going on with that <6:22:01 PM> Braden - Patient: I'm sorry what about a stoplight <6:22:03 PM> Pooja Kamath - Patient: so sorry let me um basically we've noticed that you haven't been refilling this medication as often as we kind of predicted you might need to so I was wondering if you were having any troubles with your medications <6:22:17 PM> Braden - Patient: yeah I haven't been refilling because they're just so many pills to keep track of its like ridiculous I got my sugar pill got my water pill you know and then I take one that helps me <6:22:29 PM> Pooja Kamath - Patient: here <6:22:31 PM> Braden - Patient: chest pain every now and then I don't know if I'm having a heart attack or not so half the time I'm taking it hoping that I don't die and then I take one for my blood pressure on top of that and then that I mention I think I take a sugar thing so it's just it's ridiculous like why this is not okay what can we not get me down to like one or two <6:22:46 PM> Pooja Kamath - Patient: it <6:22:54 PM> Pooja Kamath - Patient: a lot of medications can be very stressful you know something you can it can be really tedious and we don't want to burden you <6:22:54 PM> Pooja Kamath - Patient: with <6:23:05 PM> Braden - Patient: here <6:23:06 PM> Pooja Kamath - Patient: having to keep track of all this but <6:23:10 PM> Pooja Kamath - Patient: hmm yeah but what do you see as the benefit of taking these medications even though they are numerous what are some good things that do come out of it <6:23:21 PM> Braden - Patient: honestly that's all on your side because I don't know I don't take my blood pressure and stuff but it's supposed to get better but it feels like every time I take one something goes wrong I take my water pill and all I do is pee all day I take my pain pill then I can't poop and then I take you know the <6:23:40 PM> Braden - Patient: for my neuropathy and then I get so tired but I have to go to work <6:23:40 PM> Braden - Patient: so <6:23:44 PM> Pooja Kamath - Patient: mmm <6:23:46 PM> Braden - Patient: or <6:23:46 PM> Braden - Patient: at this point I feel like are we just sitting and wishing for the best <6:23:46 PM> Pooja Kamath - Patient: mm-hmm well that is that takes teamwork between both of us to see what is going to work the best for you so what medications are able to take consistently right now <6:23:51 PM> Braden - Patient: we change something <6:24:07 PM> Braden - Patient: I try to take the blood pressure one because I have felt better on it <6:24:14 PM> Braden - Patient: it my swelling has gone down <6:24:17 PM> Braden - Patient: now I don't like that I have to pee every five minutes with that but whatever we'll deal with that one <6:24:24 PM> Braden - Patient: I take the chest pain one under my tongue just for one I need it <6:24:29 PM> Braden - Patient: doesn't do anything I don't know so half the time I don't take it you know <6:24:33 PM> Braden - Patient: probably just people trying to take my money there <6:24:36 PM> Braden - Patient: the back pain one <6:24:36 PM> Braden - Patient: I mean <6:24:40 PM> Braden - Patient: need to use my back so <6:24:42 PM> Braden - Patient: I would like to stay on that one <6:24:45 PM> Braden - Patient: it would be real nice if we could like combine to like maybe there's one for the back pain and the neuropathy that could be one pill instead of two or something like that <6:24:52 PM> Pooja Kamath - Patient: mm-hmm <6:24:55 PM> Pooja Kamath - Patient: kind <6:24:55 PM> Pooja Kamath - Patient: yeah and they do have a couple and there's a couple options for combining pills we do <6:25:05 PM> Pooja Kamath - Patient: insurance <6:25:05 PM> Pooja Kamath - Patient: I'm trying to prescribed Things based on what insurance will cover so we're it's you and me were partnership but we're also <6:25:13 PM> Pooja Kamath - Patient: like to poke their head in and see what we're doing and and cost-wise I have a combination can sometimes like certain combinations can sometimes be more expensive <6:25:13 PM> Pooja Kamath - Patient: but <6:25:27 PM> Pooja Kamath - Patient: what we <6:25:27 PM> Pooja Kamath - Patient: yeah let's take a look at <6:25:31 PM> Pooja Kamath - Patient: of here and see what we can kind of combine <6:25:32 PM> Braden - Patient: yeah because at this point I we just need to change some of that I can't keep doing this this is frustrating it's not even the money it's the time to go get all these and having to remember to take these every day <6:25:36 PM> Pooja Kamath - Patient: okay <6:25:42 PM> Pooja Kamath - Patient: mmm <6:25:44 PM> Pooja Kamath - Patient: mm-hmm <6:25:50 PM> Pooja Kamath - Patient: and <6:25:50 PM> Pooja Kamath - Patient: yeah <6:25:52 PM> Pooja Kamath - Patient: and I think that's something that a lot of people struggle with because <6:25:58 PM> Pooja Kamath - Patient: you know you're busy <6:25:58 PM> Pooja Kamath - Patient: it's <6:26:01 PM> Pooja Kamath - Patient: like you've got a lot going on at work with kids and family <6:26:05 PM> Pooja Kamath - Patient: and you know we we want you to be able to prioritize your own health without it being such an important to you and one thing that <6:26:16 PM> Pooja Kamath - Patient: do kind of you know talk about with patients is that if we can <6:26:22 PM> Pooja Kamath - Patient: control some of these things with diet and lifestyle modifications I'm really open to that option and kind of knocking some pills off so what <6:26:33 PM> Pooja Kamath - Patient: and and with that so the biggest <6:26:33 PM> Pooja Kamath - Patient: not <6:26:37 PM> Pooja Kamath - Patient: easy but the biggest thing that we can do to to be able to start taking off pills and to start feeling better is kind of maybe losing some weight so how do you feel about that <6:26:51 PM> Braden - Patient: I mean it's not something that I want to do because <6:26:54 PM> Pooja Kamath - Patient: mmm <6:26:55 PM> Braden - Patient: feel like I'm already so busy that I got a <6:26:58 PM> Braden - Patient: you know now I have to try to find different food and stuff but with that this might seem stupid of me to ask but should I try one of them <6:27:09 PM> Braden - Patient: pick shots to add the medication now so we can stop more medication down the line <6:27:12 PM> Pooja Kamath - Patient: well there's a lot of options but <6:27:22 PM> Pooja Kamath - Patient: with with the you know the complexity and this this stress of like already taking so many medications my first go to probably wouldn't be adding another medication because you know that I can get frustrating and then you know that's a whole new set of side effects or things we have to kind of watch out for so my first go-to might not be that but again it's it's you and me were a partnership in this so what sounds <6:27:50 PM> Braden - Patient: well you know my friend Carol she had a heart attack last year she said 66 God bless her and she was able to get on one of those fat shots too and she's doing just fine and they lowered her her diabetes medication and I think that would be good <6:27:52 PM> Pooja Kamath - Patient: what is something that sounds favorable to you <6:27:56 PM> Pooja Kamath - Patient: mmm <6:28:02 PM> Pooja Kamath - Patient: here <6:28:08 PM> Pooja Kamath - Patient: mmm <6:28:15 PM> Pooja Kamath - Patient: yeah <6:28:17 PM> Pooja Kamath - Patient: for those types of medications <6:28:21 PM> Pooja Kamath - Patient: first how do you feel about <6:28:26 PM> Pooja Kamath - Patient: you know getting into a good regimen with your current medications and potentially some Lifestyle Changes <6:28:32 PM> Braden - Patient: am I not in again regimen right now I got all the pills I thought that's what we've been doing <6:28:33 PM> Pooja Kamath - Patient: mm-hmm you have all the right deals but it's not going to help you to its full potential unless you know we can figure out a way to make it easier for you to take all of them <6:28:33 PM> Pooja Kamath - Patient: and <6:28:52 PM> Pooja Kamath - Patient: oh <6:28:52 PM> Pooja Kamath - Patient: oh yeah <6:28:55 PM> Pooja Kamath - Patient: okay yeah so it sounds like we talked a lot about a lot today <6:29:02 PM> Pooja Kamath - Patient: and different options what do you think the next best step is for us today <6:29:03 PM> Angie: all right I'll stop you here <6:29:08 PM> Pooja Kamath - Patient: okay ________________________________________________________________________________ End of transcript at Apr 28, 2025, 6:29:10 PM
