Patient: like <1:52:58 PM> Ryan_Doctor: all right Graham looks like last time you came for your visit we upped your dose to 40 milligrams of Prozac so she told me how that's working for you <1:53:12 PM> Graham - Patient: yeah I mean it's definitely helped a lot from being on the 20 milligrams since we've upped it up but I'm definitely still having some symptoms if you still feel like <1:53:21 PM> Graham - Patient: tired all the time feel like I can't sleep very much <1:53:26 PM> Graham - Patient: he like my appetitus still kind of decreased like it was last time <1:53:32 PM> Graham - Patient: and I feel like <1:53:35 PM> Graham - Patient: I just have <1:53:37 PM> Graham - Patient: so little energy that is kind of impacted work feel like I'm not able to get as much done at work <1:53:44 PM> Graham - Patient: yeah it's definitely better but I <1:53:47 PM> Graham - Patient: don't want to <1:53:47 PM> Graham - Patient: see <1:53:48 PM> Graham - Patient: improvements <1:53:48 PM> Ryan_Doctor: okay well <1:53:52 PM> Ryan_Doctor: but <1:53:52 PM> Ryan_Doctor: it looks like since we increased it the last time we're up to 40 milligrams right so in the last visit we kind of said you know this is probably going to be the maximum dose we're going to hit <1:54:03 PM> Ryan_Doctor: but have you seen any Improvement since we've increased it <1:54:05 PM> Graham - Patient: yes in some improvement like I don't feel guilt towards things like I used to anymore <1:54:08 PM> Ryan_Doctor: hm and how many times a week would you say you're having these symptoms <1:54:14 PM> Graham - Patient: I don't feel like I'm in a fog or anything like that anymore like a moving slow throughout the day so that's definitely improved feel like I can concentrate a little better but just between having trouble sleeping and not eating as much and having so little energy I feel like that's still impacting work <1:54:30 PM> Graham - Patient: mostly on Mondays and Tuesdays I feel like I've just <1:54:35 PM> Ryan_Doctor: and how has that changed since the last time we were on 20 milligrams <1:54:38 PM> Graham - Patient: get <1:54:38 PM> Graham - Patient: once I get back to sleeping on the weekend I can finally sleep in a little bit and feel a little bit better rested but then I <1:54:45 PM> Graham - Patient: work at the beginning of the week and it's just so exhausting but once I get going again through the week it's better <1:54:51 PM> Graham - Patient: here and say Well it started out it was pretty much every single day of the week and then on 20 milligrams it was probably every day except Friday <1:54:51 PM> Graham - Patient: because <1:55:03 PM> Graham - Patient: a you know it's the end of the work we get something to look forward to the weekend so probably just Monday through Thursday on the 2 0 <1:55:10 PM> Graham - Patient: are definitely moving in the right direction just being it Monday and Tuesday now on the 40 but still want to have to get rid of all that <1:55:11 PM> Ryan_Doctor: well that's good at least I mean at least we're seeing some improvement right <1:55:11 PM> Ryan_Doctor: you know <1:55:17 PM> Graham - Patient: yeah I agree with that I think we're definitely moving in the right direction <1:55:21 PM> Ryan_Doctor: going somewhere <1:55:22 PM> Ryan_Doctor: yeah we're moving the right direction now I know you had some stuff going on in the last visit some some personal stuff is anything kind of changed anything showed up in these past couple of weeks <1:55:27 PM> Graham - Patient: that really is just been all the same stuff with words you know <1:55:38 PM> Ryan_Doctor: you know <1:55:42 PM> Graham - Patient: impacted my ability to <1:55:45 PM> Graham - Patient: get all my work done I feel like I'm just so tired that <1:55:49 PM> Graham - Patient: to take a nap during the day but you know it works I can't do that <1:55:53 PM> Graham - Patient: um but I just feel like I'm able to get as much stuff done so it's definitely still impacting work <1:55:54 PM> Ryan_Doctor: but <1:55:55 PM> Ryan_Doctor: okay yeah you said you said you usually the weekends you're feeling pretty good right like you can catch up on sleep in the work weeks over <1:56:02 PM> Graham - Patient: yeah because I can take naps during the day on the weekends I can feel a little more refreshed refreshed and catch up on sleep that way so that definitely makes me feel better but I obviously can't do that throughout the week <1:56:06 PM> Ryan_Doctor: mm-hmm is there anything that makes you feel worse is it like work in general or <1:56:20 PM> Graham - Patient: now works great honestly it's a great work environment like I said it's just the <1:56:25 PM> Ryan_Doctor: mm-hmm <1:56:31 PM> Graham - Patient: deep in the appetite and kind of lack of energy that kind of gets in the way that but definitely able to concentrate better and on the weekends I really feel fine because I'm able to get that nap in <1:56:31 PM> Graham - Patient: but <1:56:42 PM> Graham - Patient: usually do that from like one to three so that's kind of right in the afternoon for work when I got to be getting stuff done <1:56:47 PM> Ryan_Doctor: and what about <1:56:50 PM> Graham - Patient: yeah I'd say so still haven't some some of those same symptoms throughout the week when I get home just because <1:56:50 PM> Ryan_Doctor: home like when you get home from work everything going well there <1:56:58 PM> Ryan_Doctor: mm-hmm is anyone at home with you or is it just you <1:57:02 PM> Graham - Patient: like I'm tired at work all day and <1:57:05 PM> Graham - Patient: feel like I try to eat but sometimes it still can't so <1:57:09 PM> Graham - Patient: trying to sit and do that versus just going straight to bed every day after work <1:57:14 PM> Graham - Patient: uh well my roommate just moved out <1:57:17 PM> Ryan_Doctor: mmm <1:57:21 PM> Graham - Patient: so <1:57:23 PM> Graham - Patient: than one change so I feel like I don't have <1:57:27 PM> Graham - Patient: anybody to talk to as much now at least about life or just regular stuff and then we got a upcoming interview that I'm a little worried about wanna definitely do well on that but these symptoms are <1:57:38 PM> Ryan_Doctor: Wonder roommate end up moving out <1:57:39 PM> Graham - Patient: kind of <1:57:40 PM> Graham - Patient: of an obstacle in front of that <1:57:42 PM> Graham - Patient: like a couple weeks ago <1:57:44 PM> Ryan_Doctor: a couple weeks ago <1:57:46 PM> Graham - Patient: it's a weirdly very close we lived together for a while <1:57:47 PM> Ryan_Doctor: okay you guys pretty tight <1:57:55 PM> Graham - Patient: um we do each other really well and <1:57:58 PM> Ryan_Doctor: yes still talk much like you guys text often or do you end up moving to <1:57:58 PM> Graham - Patient: this is a good person to talk to for both of us to kind of unwind after work and just catch up and talk about something besides work for once <1:58:07 PM> Graham - Patient: he moved a little further away so I don't see him as much he still text him here and there but he's busy with work now too so <1:58:20 PM> Graham - Patient: he's got his new job and I'm working on getting my new job so we'll kind of see how it plays out from there <1:58:21 PM> Ryan_Doctor: yeah it's probably hard to you know have this conversations you guys used to have in that you know he's not living there anymore <1:58:28 PM> Graham - Patient: yeah it's a little different from text and trying to find time to call versus living in the same house so they're always right there to talk to <1:58:41 PM> Ryan_Doctor: yeah sometimes texting is just doesn't feel the same because it's like well you're in person right you can you can start going back and forth and getting ideas and stuff over at Texas just the dot and you send it <1:58:44 PM> Graham - Patient: there there's definitely a lot less <1:58:56 PM> Ryan_Doctor: this is <1:58:59 PM> Graham - Patient: emotion and personality with texting because you know it's every interpreted versus how they're saying it right in front of you <1:59:05 PM> Ryan_Doctor: yeah how did that make you feel whenever you're like just chatting with them you know in person versus text <1:59:14 PM> Graham - Patient: I'd say texting definitely still helps but it you know not nearly as much obviously kind of like for the reasons we just talked about <1:59:17 PM> Ryan_Doctor: mm-hmm you have anyone else you can talk to <1:59:21 PM> Graham - Patient: yeah I got some family but they live kind of far away so I don't see them a whole lot <1:59:30 PM> Graham - Patient: um you know the past year to my roommates honestly been the big one to talk to <1:59:30 PM> Ryan_Doctor: mm-hmm so your roommate looks like your roommate moved out a couple of weeks ago but what about this work thing tell me a little bit more about that <1:59:38 PM> Graham - Patient: with my current job or my upcoming one <1:59:44 PM> Ryan_Doctor: you know your upcoming one the one you got the interview for right <1:59:48 PM> Graham - Patient: yeah so I got an interview in the next week or two <1:59:54 PM> Graham - Patient: to be a big step up for my current job it'll still be kind of close by as well have to really go anywhere but <2:00:01 PM> Graham - Patient: you know it's a big deal to have a new job and <2:00:05 PM> Graham - Patient: it'll be a better job so it's something that I really want I want to be able to be focused and work hard when I get there and not have to deal with <2:00:14 PM> Graham - Patient: symptoms so <2:00:16 PM> Graham - Patient: you know increasing my dose from last time definitely helped I still want to kind of get this resolved sooner rather than later so that I can at least be another step closer from the right direction before this new job starts if I get it <2:00:18 PM> Ryan_Doctor: and how's this new job interview as this making you feel and make you feel a little like or a little bit like let's go let's do it kind of thing <2:00:29 PM> Graham - Patient: it definitely more anxious than anything can I just talked about you know I don't want to be having these symptoms when I start my new job because I've seen how it impacts my current job and if I start my job out like that I'm worried I'm not going to be able to keep the job so <2:00:37 PM> Ryan_Doctor: yeah is this one of those things that you'd like to have your roommate to talk to about <2:00:52 PM> Graham - Patient: yeah I mean it's always good to have somebody to talk to about stuff like that <2:00:56 PM> Ryan_Doctor: mm-hmm <2:01:03 PM> Graham - Patient: but since he moved out he's just not as available to talk to like it used to be and kind of vice versa so I feel like we were that person for each other <2:01:11 PM> Graham - Patient: yeah definitely <2:01:11 PM> Ryan_Doctor: well you know I'm I'm glad that the 40 Megs is working out pretty well at least it's seems to be having some improvement right we still got the stuff going on <2:01:21 PM> Ryan_Doctor: yeah I know we talked about the last time we talked about doing a <2:01:25 PM> Graham - Patient: oh yeah I thought about it but I hadn't made an appointment yet just because still feeling so tired every day after work I feel like I just kind of want to go home and try and eat something even though I don't have too much of an appetite <2:01:26 PM> Ryan_Doctor: our referral to like therapy you thought about that at all <2:01:30 PM> Ryan_Doctor: mm-hmm <2:01:44 PM> Graham - Patient: trying to catch up on some sleep so <2:01:45 PM> Ryan_Doctor: yeah but did you when you were talking to your roommate when he was still there did you ever find yourself not being as tired <2:01:47 PM> Graham - Patient: feel like I'm barely even able to do the basic things of life I don't want to add an extra <2:01:53 PM> Graham - Patient: or two dependent on if it's virtual and it's an hour or however long it is or if I have to drive somewhere and take even more time out of my day when I'm already so tired at the end of the day I don't know that I get too much out of it <2:02:06 PM> Graham - Patient: oh yeah definitely <2:02:15 PM> Graham - Patient: say it's been more in the past couple weeks since we moved out <2:02:19 PM> Graham - Patient: after of my dose that I've been feeling symptoms just because I feel like he was such a good person to talk to <2:02:20 PM> Ryan_Doctor: yeah so it's starting to sound like a little bit like you might just really need someone to talk to so if you're saying that you had you didn't have these symptoms as bad when your roommate was still living with you and you're able to talk about stuff <2:02:40 PM> Ryan_Doctor: maybe finding a therapist might be a good way to <2:02:44 PM> Ryan_Doctor: good way to get rid of some of those symptoms especially if we're already at 40 migs <2:02:50 PM> Graham - Patient: yeah I think that could be a good idea <2:02:53 PM> Graham - Patient: um seeing that he was such a good person to talk to and worried about talking to somebody new and having to start over because since he's been with me the past couple years he's got <2:02:58 PM> Ryan_Doctor: oh yeah <2:03:04 PM> Graham - Patient: a pretty good handle on <2:03:06 PM> Graham - Patient: everything I've got going on just because we talked about it every single day <2:03:10 PM> Ryan_Doctor: well you know it's it's kind of hard with with therapy too because a lot of people think that it's you know it's an immediate fix but I think a lot of people forget that some of the best parts about therapies developing that relationship it comes with time right so your roommate already knew you right because you guys have been living together for so long <2:03:30 PM> Ryan_Doctor: um a therapist a trained therapist can get to that point as well <2:03:35 PM> Ryan_Doctor: to do <2:03:35 PM> Ryan_Doctor: just it's gonna take time but I think that it really you could see a lot of benefits with being able to talk to someone about not only this roommate change and maybe this interview coming up but your ability <2:03:50 PM> Ryan_Doctor: in the future as well if something new came up you might have a good opportunity to discuss with the therapist ________________________________________________________________________________ End of transcript at Apr 29, 2025, 2:04:00 PM
