Doctor: okay I understand you were here three months ago or six months ago just wanted to clear that up <2:45:13 PM> Tate Barney - Patient: here six months ago we started me on we'll just go search <2:45:24 PM> Tate Barney - Patient: it's Citalopram we'll go with that one we started me on escitalopram six months ago and then three months ago we upped my dose so I'm just back for a check-in because we upped my dose <2:45:26 PM> Rahul Manne - Doctor: okay <2:45:28 PM> Rahul Manne - Doctor: okay how have you been doing since we up here dose of the medication <2:45:35 PM> Tate Barney - Patient: doing okay I mean still pretty tired I'm not really motivated to do a whole lot still <2:45:40 PM> Rahul Manne - Doctor: here <2:45:48 PM> Tate Barney - Patient: works not going so great for me <2:45:52 PM> Tate Barney - Patient: you know just a couple issues I feel a little bit better on the medication but not where I not where I want it to be <2:45:59 PM> Rahul Manne - Doctor: okay sounds like you feel a little bit better but you sounds like work is a source of stress <2:46:00 PM> Tate Barney - Patient: it's just <2:46:06 PM> Rahul Manne - Doctor: okay um what stressful about work is there a specific aspect or what's going on there <2:46:13 PM> Tate Barney - Patient: well we get performance evaluations every couple of weeks and mine have come back you know I'm not performing where I need to be with my other colleagues so it's it's been a little bit stressful trying to figure out how to get back in and you know up my performance at work <2:46:24 PM> Rahul Manne - Doctor: it okay okay <2:46:30 PM> Tate Barney - Patient: it works fine I just feel like I'm so tired all the time and I just don't have the motivation to get as much done as my other colleagues do so I'm not sure if the medications working or not from that aspect <2:46:33 PM> Rahul Manne - Doctor: are you why do you think that you're not performing on the level of your colleagues is is the workplace a healthy environment like what are some possible reasons you can think of <2:46:54 PM> Rahul Manne - Doctor: okay okay <2:47:00 PM> Rahul Manne - Doctor: okay you mentioned being tired all the time and lack of motivation how have you been doing with sleep <2:47:03 PM> Tate Barney - Patient: okay sleep soon okay it's hard to follow asleep at night but then once I'm asleep I'm pretty good about staying asleep <2:47:09 PM> Rahul Manne - Doctor: mm-hmm <2:47:17 PM> Tate Barney - Patient: um I go to bed at like 11:00 and then I'm up for work about 6 a.m. by the time I fall asleep it's usually like right around midnight <2:47:24 PM> Rahul Manne - Doctor: okay okay and how long have you been feeling this way having trouble with sleep and lack of motivation and stuff like that <2:47:27 PM> Tate Barney - Patient: it's probably a year and then I finally wanted to do something about it so I came into the office six months ago and we tried that medication so it's helped a little bit but you know I'm still looking for ways to <2:47:38 PM> Rahul Manne - Doctor: mm-hmm <2:47:47 PM> Rahul Manne - Doctor: yeah so I understand we did start the medication you've seen some improvement have you tried anything else any non-medication related like interventions support groups or counseling anything of that sort <2:47:49 PM> Tate Barney - Patient: to make some more progress I'm kind of tired of feeling like this <2:48:08 PM> Tate Barney - Patient: I know you guys recommended counseling to me at the last couple of office visits but no I haven't done anything with it <2:48:09 PM> Rahul Manne - Doctor: okay <2:48:16 PM> Tate Barney - Patient: oh I don't know I kind of feel weird about going to feels weird to talk to a stranger about my problems so I don't know it's just kind of hard to find somebody that <2:48:18 PM> Rahul Manne - Doctor: what seems to be is there something is there a barrier that's preventing you from going to counseling <2:48:26 PM> Rahul Manne - Doctor: why is that something that you'd be interested in <2:48:30 PM> Rahul Manne - Doctor: mm-hmm <2:48:41 PM> Tate Barney - Patient: you know I feel like I could connect really well with so I just I'm not really sure about about the counseling and like talking to a stranger about my problems <2:48:51 PM> Rahul Manne - Doctor: yeah I I can totally understand that I'm you know you seem it obviously seems like you're going through a lot with a lack of motivation the stress about the the job upcoming performance evaluation so that definitely is a source of stress combined with the fact that you're not sleeping as well and on the surface yeah talking to someone that you've never met before can seem a little daunting at first <2:49:02 PM> Tate Barney - Patient: well I used to talk to my roommate about stuff but she just moved out a couple of months ago so <2:49:02 PM> Tate Barney - Patient: there's <2:49:17 PM> Rahul Manne - Doctor: think you have that apprehension of <2:49:20 PM> Rahul Manne - Doctor: I guess maybe reaching out and talking to someone <2:49:33 PM> Tate Barney - Patient: then you know I'm just by myself in the apartment so it's a little bit quieter <2:49:34 PM> Rahul Manne - Doctor: mm-hmm <2:49:39 PM> Tate Barney - Patient: I don't know I don't know why I'm so apprehensive to to go ahead with the counseling <2:49:44 PM> Tate Barney - Patient: no not really just these these visits that I've had at the the doctor about my medication you guys are kind of the only ones outside of my roommate that know about what's going on <2:49:44 PM> Rahul Manne - Doctor: um so I understand that you mentioned your little bit apprehensive to talk to a stranger about your problems is there anyone and also mentioned that you were talking to your roommate about some of the things you are going through and she's obviously moving out is there anybody else in your life that in the meantime that you've talked to about some of the things you're struggling with <2:50:04 PM> Rahul Manne - Doctor: okay <2:50:16 PM> Rahul Manne - Doctor: why do you think your roommate was the only person that you are comfortable confiding in <2:50:20 PM> Tate Barney - Patient: here we kind of grew up together so I've known her for a really long time we went to college together and got our first jobs out of college together <2:50:29 PM> Rahul Manne - Doctor: okay <2:50:33 PM> Tate Barney - Patient: but she decided to move out she got a different job in another city so she left but it's just feeling comfortable and familiar with somebody I think is the main thing for me <2:50:34 PM> Rahul Manne - Doctor: okay okay is there anybody at work that you have that level of comfort with <2:50:43 PM> Tate Barney - Patient: and <2:50:43 PM> Tate Barney - Patient: it's not really especially with my performance evaluations and everything I feel like my boss is mad at me everybody else is kind of mad at me for not meeting quotas <2:50:49 PM> Rahul Manne - Doctor: okay okay sounds like there's a lot going on there <2:50:59 PM> Tate Barney - Patient: like that like I'm just not as productive at work so I feel like they wouldn't be as receptive to to hearing what I have to say at work <2:51:11 PM> Rahul Manne - Doctor: if I were to give you a scale from zero one to ten one being you're not you're not willing to talk at all just to like a counselor look at <2:51:11 PM> Rahul Manne - Doctor: like other <2:51:24 PM> Rahul Manne - Doctor: non-medication related interventions to 10 meaning that you would be very open to undergoing those sort of things what would you say you fall on the scale <2:51:34 PM> Tate Barney - Patient: probably around a six I would be willing to talk to somebody if you know we kind of clicked right away I've heard some people have to shop around for therapists and things like that do you guys have any recommendations for therapists that you've talked to people about or counseling services that other patients have really liked in the past <2:51:54 PM> Rahul Manne - Doctor: we do have a list of services that we recommended patients to and that's definitely something that we would love to give you today so you mentioned being a six <2:52:10 PM> Rahul Manne - Doctor: why do you think that number isn't higher or lower what makes that number a 6 for you <2:52:16 PM> Rahul Manne - Doctor: uh-huh <2:52:18 PM> Tate Barney - Patient: I think it's more fear being embarrassed talking about this I know a lot of we hear a lot of other people have the same problems <2:52:25 PM> Rahul Manne - Doctor: okay but <2:52:28 PM> Tate Barney - Patient: but I don't want to feel judged by somebody it's kind of scary sharing your life story with somebody else so I think it's the fear of the unknown that's probably not leaning me more towards a higher number <2:52:41 PM> Paul Hershberger: I'll go ahead and stop you here <2:52:44 PM> Rahul Manne -
