doctor: you know how are you doing today <5:46:10 PM> Gus Miller - patient: I I haven't been taking some of my metformin recently it's always <5:46:10 PM> Gus Miller - patient: always <5:46:17 PM> Gus Miller - patient: gives me some upsets my my intestinal tract you know and <5:46:22 PM> Gus Miller - patient: like the other medications they just they really make it tough to just live man <5:46:28 PM> Young Jun Park - doctor: okay so it sounds like you're pretty frustrated about you know taking so many medications and you feel like it's not working <5:46:37 PM> Gus Miller - patient: yeah well um I mean overall health in general I mean hasn't changed a whole lot and I I don't feel like the muds the meds really make me much healthier I mean <5:46:38 PM> Young Jun Park - doctor: would be okay if we took a minute to kind of talk about how you feel about these medications and kind of your overall health in general <5:46:56 PM> Young Jun Park - doctor: it's <5:47:02 PM> Gus Miller - patient: I wouldn't say necessarily feel better when I'm taking them in fact I probably feel worse because <5:47:09 PM> Gus Miller - patient: get a bunch of diarrhea from the metformin I take that <5:47:13 PM> Gus Miller - patient: that a tour of a Stan and it just makes my muscles ache and I just feel much better honestly when I'm not taking the medication so I don't <5:47:26 PM> Gus Miller - patient: I don't really know why why they're beneficial you know <5:47:29 PM> Young Jun Park - doctor: okay so I can see I can hear that there are some side effects that you're having with these medications <5:47:41 PM> Young Jun Park - doctor: in terms of your lifestyle how has that been going <5:47:43 PM> Gus Miller - patient: um <5:47:50 PM> Gus Miller - patient: I've been Lifestyles been saying my <5:47:54 PM> Gus Miller - patient: go to work nine-to-five come home <5:47:56 PM> Young Jun Park - doctor: okay and so <5:47:58 PM> Gus Miller - patient: a couple beers watch some TV go to bed kind of repeat <5:48:05 PM> Gus Miller - patient: wish wash throughout the week you know <5:48:11 PM> Gus Miller - patient: ah <5:48:11 PM> Young Jun Park - doctor: it's a kind of you know I kind of want to ask you you know currently in your life right now what would you say is the most important thing in terms of your health <5:48:23 PM> Young Jun Park - doctor: this <5:48:28 PM> Gus Miller - patient: I guess probably the diabetes you know I <5:48:34 PM> Gus Miller - patient: I have a hard time feeling my feet and stuff I I see the foot doctor about once a year so <5:48:45 PM> Gus Miller - patient: I've <5:48:45 PM> Gus Miller - patient: I'm starting to get some of that neuropathy but I mean I've <5:48:51 PM> Gus Miller - patient: of these medications for United still get the neuropathy so it's almost like why I put myself through all this hell of all the men if I'm still gonna still gonna develop all the complications you know <5:49:05 PM> Young Jun Park - doctor: mm-hmm yeah so I can hear that you're pretty frustrated about things that are just kind of going and they're progressing even though you're taking these and that's you know one thing that we definitely do want to do is take you off the metformin because if you're experiencing these side effects we definitely don't want you to have these side effects <5:49:24 PM> Young Jun Park - doctor: but from a scale of one to ten one being like you know you're not ready to make a change at all versus 10 meaning that like you're very ready to make some kind of changes on what scale are like on that scale what would you be <5:49:40 PM> Young Jun Park - doctor: okay and why is it a three or four why not like a 01 or two <5:49:41 PM> Gus Miller - patient: about a three or four <5:49:45 PM> Gus Miller - patient: I mean I like to be healthier you know like I said I have the neuropathy setting in <5:50:01 PM> Gus Miller - patient: ah sometimes like I <5:50:04 PM> Gus Miller - patient: I <5:50:04 PM> Gus Miller - patient: you know I can't really play with the grandkids because you know getting down to my hands and knees it hurts my joints I feel a little out of breath sometimes and <5:50:18 PM> Gus Miller - patient: to be healthier but I just <5:50:21 PM> Gus Miller - patient: don't know how you know I <5:50:24 PM> Gus Miller - patient: mean I've had diabetes and high blood pressure for so long and <5:50:30 PM> Gus Miller - patient: from what <5:50:30 PM> Gus Miller - patient: you know even when I take my medications it doesn't doesn't seem to improve at least <5:50:36 PM> Gus Miller - patient: I've seen and then I stopped taking the medications because I don't feel like they're doing anything and <5:50:44 PM> Gus Miller - patient: um but I feel like it doesn't make much of a difference either way but <5:50:47 PM> Young Jun Park - doctor: and when you were not taking the medication beforehand kind of what was the progression like <5:50:49 PM> Gus Miller - patient: that I don't know <5:50:57 PM> Gus Miller - patient: sometimes it just missed doses you know if I don't feel like taking my high blood pressure medication that day I'll just not take it <5:51:10 PM> Gus Miller - patient: it happens about half the time <5:51:13 PM> Young Jun Park - doctor: okay <5:51:13 PM> Young Jun Park - doctor: and <5:51:15 PM> Young Jun Park - doctor: in a hypothetical situation if you were to take this you know kind of pill that where you are able to help you know kind of work towards your diabetes what kind of things would you know you do differently what would that enable you to do <5:51:27 PM> Gus Miller - patient: plate ah <5:51:36 PM> Gus Miller - patient: I mean if I really it comes down to family I said I have some young grandkids and <5:51:44 PM> Gus Miller - patient: I really wish I was more more mobile <5:51:48 PM> Gus Miller - patient: and you know like if I could take myself back 20 years when <5:51:55 PM> Gus Miller - patient: when I was able to get out my hands and knees and everything and <5:51:59 PM> Gus Miller - patient: and play with my kids when they were young I wish I had those but <5:52:03 PM> Young Jun Park - doctor: right yeah for sure it sounds like you've been through a lot and you're really frustrated but it sounds like you know your grandchildren are you know people that can motivate you so you know during our talk you know if there's one thing that you could change whether it be the medication like changes that we could do diet what's one very doable thing that we can work on right after this visit <5:52:04 PM> Gus Miller - patient: you know I I really don't like taking a lot of these meds but I think maybe maybe I could try to just <5:52:46 PM> Young Jun Park - doctor: the <5:52:52 PM> Gus Miller - patient: a little less especially at dinner time you know maybe try and not go up for that third plate you know keep it to one to two plates that might cut back on some of the calories at least <5:53:01 PM> Young Jun Park - doctor: inside <5:53:07 PM> Young Jun Park - doctor: okay <5:53:08 PM> Young Jun Park - doctor: yeah I think that sounds like a great plan um and so I just kind of want to summarize our visit so you know it sounds like you feel very overwhelmed about everything about all the medications and you feel frustrated that it's not working but we you know for our little step forward what we're going to do is we're going to work on kind of improving our diet kind of eating like a little bit less and something that's you said that you're able to control and so you know whatever it is you know I'm here to support you and I'd like to check in on you again if that's okay with you <5:53:43 PM> Gus Miller -
