Patient joined the conference <1:35:50 PM> Kevin - Doctor joined the conference <1:35:58 PM> Kevin - Doctor: hey um I heard you say that you were a little bit frustrated with your medication tell me more about that <1:36:12 PM> Paul Hershberger - Patient: gosh yes seems like <1:36:16 PM> Paul Hershberger - Patient: maybe not every time but many of the times I've come into to see you or any other doctor it's like <1:36:23 PM> Paul Hershberger - Patient: I don't <1:36:23 PM> Paul Hershberger - Patient: we're going to add this medication and <1:36:26 PM> Paul Hershberger - Patient: know it's just a pile of pills and <1:36:26 PM> Paul Hershberger - Patient: I can't <1:36:29 PM> Paul Hershberger - Patient: Sam all that <1:36:31 PM> Paul Hershberger - Patient: or they'll help me in any sort of way <1:36:36 PM> Kevin - Doctor: a benefits have you seen with taking them <1:36:40 PM> Paul Hershberger - Patient: that that's a great question <1:36:44 PM> Paul Hershberger - Patient: you know I <1:36:47 PM> Paul Hershberger - Patient: I I guess I hope that <1:36:51 PM> Paul Hershberger - Patient: Health hasn't gotten worse because I'm taking these meds but I don't know that I feel any better in fact sometimes I think I may feel better on days when I don't take them <1:37:04 PM> Kevin - Doctor: hear you and I'm sure that could <1:37:08 PM> Kevin - Doctor: possibly be a little bit frustrating to you know not know if you can see directly the impacts of the medicine you're taking <1:37:09 PM> Paul Hershberger - Patient: um <1:37:17 PM> Kevin - Doctor: or when <1:37:17 PM> Kevin - Doctor: tell me about a day when you do feel <1:37:21 PM> Kevin - Doctor: you don't take them <1:37:25 PM> Paul Hershberger - Patient: sometimes I I forget all though <1:37:28 PM> Paul Hershberger - Patient: is probably less forgetting and moral just having the thought I don't want to take these meds today <1:37:38 PM> Paul Hershberger - Patient: so <1:37:41 PM> Paul Hershberger - Patient: I guess I sometimes feel like okay I'm <1:37:45 PM> Paul Hershberger - Patient: doing what I want not what somebody else is telling me to do so maybe that maybe that feels better <1:37:52 PM> Paul Hershberger - Patient: um I think sometimes <1:37:55 PM> Paul Hershberger - Patient: I'd say my head almost a little clearer if I'm not taking them I don't <1:38:01 PM> Paul Hershberger - Patient: it's not woozy or dizzy but <1:38:05 PM> Paul Hershberger - Patient: I don't know I don't know which medication it might be but <1:38:10 PM> Paul Hershberger - Patient: sometimes they just feels like my head's a little off so <1:38:14 PM> Paul Hershberger - Patient: it's nice when I don't feel that <1:38:17 PM> Paul Hershberger - Patient: can't say I do anything different on days whether I take them or not <1:38:18 PM> Kevin - Doctor: yeah I understand what you're saying I <1:38:26 PM> Kevin - Doctor: I understand that you might feel <1:38:29 PM> Kevin - Doctor: not want to feel that way right no one wants to feel adverse effects to their medicines <1:38:36 PM> Kevin - Doctor: what do you think it does for your body when you don't take those medicines <1:38:44 PM> Paul Hershberger - Patient: yeah <1:38:46 PM> Paul Hershberger - Patient: if <1:38:46 PM> Paul Hershberger - Patient: it well I guess I'd like to think my my body's happy to have a break but sort of the way you're asking that question <1:38:57 PM> Paul Hershberger - Patient: if they're helping me my <1:38:57 PM> Paul Hershberger - Patient: Maybe <1:38:59 PM> Paul Hershberger - Patient: bodies not liking <1:39:00 PM> Kevin - Doctor: a have you seen a <1:39:01 PM> Paul Hershberger - Patient: liking to not have them <1:39:08 PM> Paul Hershberger - Patient: um <1:39:09 PM> Kevin - Doctor: good or bad change in your blood pressure and your blood sugar when you're taking the medications first when you're not <1:39:22 PM> Paul Hershberger - Patient: well <1:39:24 PM> Paul Hershberger - Patient: if I don't take my medications I <1:39:27 PM> Paul Hershberger - Patient: typically not checking my blood sugar either and I don't <1:39:31 PM> Paul Hershberger - Patient: really check my blood pressure at home so <1:39:34 PM> Paul Hershberger - Patient: doc I don't know <1:39:34 PM> Paul Hershberger - Patient: I don't know <1:39:36 PM> Paul Hershberger - Patient: now <1:39:36 PM> Paul Hershberger - Patient: that I really know how to answer that question <1:39:40 PM> Paul Hershberger - Patient: like I knew I had this appointment today so for the last week or so I <1:39:47 PM> Paul Hershberger - Patient: take them every day figuring <1:39:50 PM> Paul Hershberger - Patient: you're a nice guy and everything but I feel like if I my blood pressure is too high or <1:39:57 PM> Paul Hershberger - Patient: number you check for my diabetes is too high then I don't know I must feel like a little kid <1:40:04 PM> Paul Hershberger - Patient: being <1:40:05 PM> Kevin - Doctor: so <1:40:05 PM> Kevin - Doctor: yeah I mean I definitely want the best for you <1:40:06 PM> Paul Hershberger - Patient: told I did something wrong <1:40:10 PM> Paul Hershberger - Patient: yeah I I know you're you're doing what's <1:40:16 PM> Kevin - Doctor: I wouldn't want it to come across as parenting more just guidance right because <1:40:16 PM> Kevin - Doctor: if <1:40:23 PM> Kevin - Doctor: outcome for you means the longer life less complications that's good for me but really it's because that's good for you <1:40:40 PM> Paul Hershberger - Patient: the science says is best for me <1:40:40 PM> Paul Hershberger - Patient: it's just <1:40:44 PM> Paul Hershberger - Patient: I don't know I'm only 60 years old and to be taking this whole pile of meds seems like <1:40:50 PM> Paul Hershberger - Patient: I don't know <1:40:51 PM> Kevin - Doctor: I mean a lot of people have to take a lot of meds so it doesn't necessarily be have to be correlated with age <1:40:53 PM> Paul Hershberger - Patient: I can really an old man and I don't want to think that I'm an old man <1:41:08 PM> Kevin - Doctor: to <1:41:08 PM> Kevin - Doctor: but is there anything that can make it easier <1:41:12 PM> Kevin - Doctor: those meds <1:41:14 PM> Paul Hershberger - Patient: a <1:41:18 PM> Paul Hershberger - Patient: I <1:41:20 PM> Paul Hershberger - Patient: when I when I use it and I use it correctly I've got a pill box so that that can make it easier <1:41:29 PM> Paul Hershberger - Patient: and when I'm <1:41:32 PM> Paul Hershberger - Patient: I guess more upset about having to take all these meds those are the times I'm less likely to <1:41:38 PM> Paul Hershberger - Patient: take the time to fill the pill box every week <1:41:42 PM> Paul Hershberger - Patient: um my wife will get on my case about it <1:41:46 PM> Paul Hershberger - Patient: so that that could make it easier <1:41:46 PM> Paul Hershberger - Patient: I think <1:41:49 PM> Paul Hershberger - Patient: gets <1:41:50 PM> Kevin - Doctor: hear what you're saying <1:41:51 PM> Paul Hershberger - Patient: I don't know that I need something <1:41:55 PM> Paul Hershberger - Patient: year it's probably more I just don't want to be doing it <1:42:01 PM> Kevin - Doctor: does your wife say once you gets frustrated <1:42:07 PM> Paul Hershberger - Patient: well she says things like you know <1:42:10 PM> Paul Hershberger - Patient: I want you around longer than you'll be if you don't take these meds <1:42:15 PM> Paul Hershberger - Patient: health <1:42:15 PM> Paul Hershberger - Patient: like I think she's concerned that my <1:42:18 PM> Paul Hershberger - Patient: get worse more quickly if if I don't take them <1:42:20 PM> Kevin - Doctor: yeah I hear you what how does that make you feel <1:42:22 PM> Paul Hershberger - Patient: um <1:42:30 PM> Paul Hershberger - Patient: well I'm not ready to <1:42:32 PM> Paul Hershberger - Patient: leave this earth either yet <1:42:36 PM> Paul Hershberger - Patient: I know she's well-meaning and all <1:42:41 PM> Paul Hershberger - Patient: I don't know I guess if I was more convinced that <1:42:48 PM> Paul Hershberger - Patient: they were really helping me in that was going to make a difference <1:42:52 PM> Paul Hershberger - Patient: because you know <1:42:55 PM> Paul Hershberger - Patient: like I said sometimes I feel a little better when <1:42:58 PM> Paul Hershberger - Patient: and it's <1:42:58 PM> Paul Hershberger - Patient: haven't taken them <1:43:00 PM> Paul Hershberger - Patient: like when I take them it makes me feel <1:43:04 PM> Paul Hershberger - Patient: 8 so <1:43:06 PM> Paul Hershberger - Patient: you know if you've got a headache and you take an Advil or something and it your headache goes away you know okay well I'm glad I took that <1:43:06 PM> Paul Hershberger - Patient: but <1:43:14 PM> Paul Hershberger - Patient: the metformin and the glibe uride <1:43:14 PM> Kevin - Doctor: it on a scale of one to ten one being no benefit 10 being will fix all your problems where do you see the role of Medicine <1:43:19 PM> Paul Hershberger - Patient: oh what three medications from my blood pressure <1:43:23 PM> Paul Hershberger - Patient: the cholesterol stuff <1:43:26 PM> Paul Hershberger - Patient: you know it's hard to hard to see the benefit <1:43:31 PM> Paul Hershberger - Patient: direct benefit <1:43:43 PM> Kevin - Doctor: that you're taking <1:43:47 PM> Paul Hershberger - Patient: ah <1:43:50 PM> Paul Hershberger - Patient: I'm not sure how to answer that I <1:43:50 PM> Paul Hershberger - Patient: if I <1:43:55 PM> Paul Hershberger - Patient: to answer that based on what I think you think it would be a higher number than what I think <1:44:02 PM> Kevin - Doctor: I want to hear what you think <1:44:04 PM> Kevin - Doctor: okay why not two or three <1:44:05 PM> Paul Hershberger - Patient: four maybe <1:44:08 PM> Paul Hershberger - Patient: oh well <1:44:17 PM> Paul Hershberger - Patient: I understand when <1:44:19 PM> Paul Hershberger - Patient: you've got diabetes or high blood pressure <1:44:23 PM> Paul Hershberger - Patient: or any issues with your heart <1:44:26 PM> Paul Hershberger - Patient: probably better do something so <1:44:29 PM> Paul Hershberger - Patient: taking measures I guess the most common thing to do <1:44:35 PM> Kevin - Doctor: hear you so <1:44:38 PM> Kevin - Doctor: I mean there's more than one thing I like to approach it from two different sides so your lifestyle AKA your diet your exercise and then if we need a little bit of help from some medicines to approach it from that angle too <1:44:52 PM> Kevin - Doctor: so I think that's the best approach if maybe later we can talk about some more that lifestyle approaches and then medicines but the best way to do it is to just make little chips away at both sides and then just be consistent with those changes <1:45:10 PM> Kevin - Doctor: on a scale of one to ten how much one being not any more likely 10 being <1:45:18 PM> Kevin - Doctor: going to take them every day so much more likely to take them consistently where do you fall on that scale <1:45:26 PM> Paul Hershberger - Patient: um <1:45:28 PM> Paul Hershberger - Patient: maybe today more like <1:45:32 PM> Paul Hershberger - Patient: five or a six but <1:45:35 PM> Paul Hershberger - Patient: you know to be honest with you <1:45:37 PM> Paul Hershberger - Patient: after another week or two I'm not I'm not sure <1:45:42 PM> Paul Hershberger - Patient: I don't know I'm not sure I'm gonna do much better <1:45:47 PM> Paul Hershberger - Patient: we'll stop here <1:45:48 PM> Kevin - Doctor: okay ________________________________________________________________________________ End of transcript at Jul 15, 2025, 1:45:57 PM
