Doctor: yeah it sounds like you've got a lot of things kind of going on all at once for you <1:09:58 PM> Emma Gunn - Patient: my <1:09:58 PM> Emma Gunn - Patient: just feel like it hasn't really been helping at all I just I don't know I'm worried about <1:10:04 PM> Emma Gunn - Patient: up and then recently my roommate moved out and so I'm kind of worried about how I'm gonna forward the place I don't know I'm probably gonna have to find a new roommate <1:10:04 PM> Emma Gunn - Patient: and <1:10:14 PM> Emma Gunn - Patient: just <1:10:16 PM> Emma Gunn - Patient: I don't know I just feels really hard right now <1:10:19 PM> Emma Gunn - Patient: oh yeah <1:10:24 PM> Jake Trzybinski-Viars - Doctor: yeah I can imagine that would be very stressful so hopefully we can kind of walk through this a little more and see how we can help you feel a little better and make some changes so going forward we can maybe even ease some of that stress <1:10:24 PM> Jake Trzybinski-Viars - Doctor: so <1:10:41 PM> Jake Trzybinski-Viars - Doctor: why don't you tell me a little bit more about what's going on or actually let's take a step back what would you say is the number one stressor for you right now <1:10:42 PM> Emma Gunn - Patient: oh the number one I I just I have so many right now I <1:10:50 PM> Jake Trzybinski-Viars - Doctor: mmm <1:10:56 PM> Emma Gunn - Patient: I guess I would say my work my job performance <1:10:56 PM> Jake Trzybinski-Viars - Doctor: okay um so if we <1:11:04 PM> Jake Trzybinski-Viars - Doctor: kind of stopped here said goodbye and you went on your way with the same dose of medications and no other supplemental therapies or anything else like that what do you think that would look like for you going forward these next few months until I see you again <1:11:21 PM> Jake Trzybinski-Viars - Doctor: okay um so it sounds like then maybe be open to <1:11:23 PM> Emma Gunn - Patient: I don't think anything would be better <1:11:26 PM> Emma Gunn - Patient: you know I I have that evaluation coming up and <1:11:30 PM> Emma Gunn - Patient: I'm worried that it's going to go poorly I know I kind of been struggling at work I'm just so tired all the time and <1:11:37 PM> Emma Gunn - Patient: it's hard to concentrate at work and yeah I think I do really poorly <1:11:47 PM> Jake Trzybinski-Viars - Doctor: discussing some options to <1:11:49 PM> Jake Trzybinski-Viars - Doctor: change what's going on right now and <1:11:53 PM> Jake Trzybinski-Viars - Doctor: flame not necessarily solve these problems but make some steps towards bettering your well-being <1:11:54 PM> Emma Gunn - Patient: oh yeah maybe <1:12:00 PM> Jake Trzybinski-Viars - Doctor: okay do you have any ideas before we kind of go into talk about Solutions do you have any ideas of what that may look like for you or what may be helpful for you <1:12:02 PM> Emma Gunn - Patient: why I know what our last appointments we talked about like mental health counseling I just I'm <1:12:02 PM> Emma Gunn - Patient: I'm <1:12:13 PM> Jake Trzybinski-Viars - Doctor: okay why do you think that people would be judging you <1:12:20 PM> Emma Gunn - Patient: hesitant to do that because like I'm just worried people are going to judge me <1:12:26 PM> Emma Gunn - Patient: well I I just think like <1:12:34 PM> Emma Gunn - Patient: going to counseling then people are going to know that like I have a problem or they're going to think there's something wrong with me and <1:12:42 PM> Emma Gunn - Patient: yeah I don't want I don't want people to think that about me <1:12:45 PM> Jake Trzybinski-Viars - Doctor: okay so kind of just fear of how people would be viewing you <1:12:47 PM> Emma Gunn - Patient: oh yeah <1:12:51 PM> Jake Trzybinski-Viars - Doctor: okay <1:12:54 PM> Jake Trzybinski-Viars - Doctor: just want to say that no <1:12:58 PM> Jake Trzybinski-Viars - Doctor: there's no shame in trying to seek any sort of help it sounds like you have a lot going on and it's very difficult for people to try to take on the stuff all on their own many times with financial concerns job concerns and just your feelings that you mentioned of worthlessness I think it would be really nice if we can try to make a change <1:13:21 PM> Jake Trzybinski-Viars - Doctor: on a scale of say one to ten how ready would you be right now if I said you know let's refer you to one of my colleagues a Specialists that may be able to help you further <1:13:26 PM> Emma Gunn - Patient: right now <1:13:37 PM> Emma Gunn - Patient: probably like a four <1:13:40 PM> Jake Trzybinski-Viars - Doctor: okay um what makes you say a four rather than like a one or two <1:13:40 PM> Emma Gunn - Patient: well I just I feel like I've been trying on my own and like with the medications get better and I it's just not working <1:13:45 PM> Jake Trzybinski-Viars - Doctor: mm-hmm <1:13:55 PM> Emma Gunn - Patient: so I think I do need something else I just I don't know if this is the solution or not <1:13:57 PM> Jake Trzybinski-Viars - Doctor: yeah um <1:13:57 PM> Jake Trzybinski-Viars - Doctor: and then <1:14:03 PM> Jake Trzybinski-Viars - Doctor: is there anything that you think would make you go from a four to say like a seven on being able to ready to make some sort of change whether that be seeing one of my colleagues or kind of re going through your medications and seeing if there's something we can change their <1:14:17 PM> Emma Gunn - Patient: mmm <1:14:22 PM> Jake Trzybinski-Viars - Doctor: okay <1:14:22 PM> Jake Trzybinski-Viars - Doctor: yeah <1:14:24 PM> Emma Gunn - Patient: yeah I I guess if I just knew like all my options and could decide what was the best one <1:14:32 PM> Emma Gunn - Patient: I don't know <1:14:35 PM> Jake Trzybinski-Viars - Doctor: we can definitely go through options in a little bit I am also just curious about what the support looks like for you once you leave this office <1:14:45 PM> Jake Trzybinski-Viars - Doctor: up any other colleagues friends family that may be around that would be able to lend you a hand or even just people that you're able to talk to if there's anyone because I know it's hard to just have this one meeting every three months or so is there anyone else at home or in your community that you feel is a safe person to talk to <1:14:47 PM> Emma Gunn - Patient: hey what do you mean by support <1:14:59 PM> Emma Gunn - Patient: it well at home I mean my roommate moved out so I'm I'm by myself right now I do have my cats but it's just me and then I do have family that's nearby I don't see them super often but I I could <1:15:13 PM> Jake Trzybinski-Viars - Doctor: mm-hmm <1:15:19 PM> Jake Trzybinski-Viars - Doctor: okay <1:15:31 PM> Jake Trzybinski-Viars - Doctor: um <1:15:32 PM> Jake Trzybinski-Viars - Doctor: I do think that <1:15:34 PM> Emma Gunn - Patient: mmm <1:15:34 PM> Jake Trzybinski-Viars - Doctor: haven't support in any way possible is always going to be helpful especially just if you're feeling low having that connection in that Community can be really important and we may have some resources if you'd like to kind of see what's in our local community as if there's any hobbies that <1:15:34 PM> Jake Trzybinski-Viars - Doctor: you may be <1:15:54 PM> Jake Trzybinski-Viars - Doctor: looking to get into or any places that you're hoping to kind of get your foot in the door maybe we can facilitate that and that can also help with <1:16:03 PM> Jake Trzybinski-Viars - Doctor: you know having a support group kind of less than lessening those feelings of worthlessness <1:16:10 PM> Emma Gunn - Patient: yeah I I'd be open to that <1:16:10 PM> Jake Trzybinski-Viars - Doctor: like something you may be interested in if we have resources like that <1:16:14 PM> Jake Trzybinski-Viars - Doctor: okay <1:16:19 PM> Jake Trzybinski-Viars - Doctor: and then let's see here <1:16:22 PM> Jake Trzybinski-Viars - Doctor: so ultimately whether we choose <1:16:26 PM> Jake Trzybinski-Viars - Doctor: to say <1:16:28 PM> Jake Trzybinski-Viars - Doctor: go to <1:16:28 PM> Jake Trzybinski-Viars - Doctor: medications increase of dosage of it <1:16:29 PM> Paul Hershberger: we'll go ahead and stop here <1:16:31 PM> Jake Trzybinski-Viars - Doctor: a one of my colleagues <1:16:35 PM> Jake Trzybinski-Viars - Doctor: that decision is up to you <1:16:39 PM> Jake Trzybinski-Viars - Doctor: but I do just want to reiterate that I think something would be beneficial for you and <1:16:45 PM> Jake Trzybinski-Viars - Doctor: if it's not now maybe we can take some time between now and when I see you next <1:16:51 PM> Jake Trzybinski-Viars - Doctor: just think it over see how things are going without any change <1:16:56 PM> Jake Trzybinski-Viars - Doctor: and then you can come back and see <1:16:59 PM> Jake Trzybinski-Viars - Doctor: what may be looking like the right solution for you or like I said we can kind of discuss stuff further and go from there right now <1:16:59 PM> Emma Gunn - Patient: okay yeah <1:17:08 PM> Jake Trzybinski-Viars - Doctor: okay ________________________________________________________________________________ End of transcript at Jul 21, 2025, 1:17:20 PM
