patient: it's just I haven't been performing well bosses on me just busy day-to-day stuff not doing too great <5:18:32 PM> Bethany - doctor: you know what do you mean you haven't been performing well <5:18:42 PM> Anthony - patient: to do the work <5:19:27 PM> Anthony - patient: I'm like I said the boss has been on my back <5:19:28 PM> Bethany - doctor: that been affecting you <5:19:35 PM> Anthony - patient: once the meet with me to talk about what's going on and I'm not looking forward to that <5:19:40 PM> Bethany - doctor: yeah <5:19:43 PM> Anthony - patient: so I knee really need to get this work done so I can keep my job <5:19:48 PM> Anthony - patient: oh probably not great like you said <5:19:49 PM> Bethany - doctor: do you think that conversation with your boss is going to go <5:19:58 PM> Anthony - patient: haven't been getting what I need done done the focus is just not there yeah <5:19:59 PM> Bethany - doctor: okay how's your focus improved with the medication <5:20:06 PM> Anthony - patient: um not so much still like I said my energy is improved a little bit but like my concentration isn't there <5:20:11 PM> Bethany - doctor: mmm what do you mean it's hard getting to work <5:20:19 PM> Anthony - patient: it's been tough getting to work and then when I come home I'm just tired so I can't even catch up on things from work once I get home <5:20:32 PM> Anthony - patient: I just <5:20:36 PM> Bethany - doctor: hmm <5:20:37 PM> Anthony - patient: don't want to get up in the morning don't want to hop in the car don't want to drive to work still <5:20:43 PM> Anthony - patient: I'm gonna get to work like I said I'm just so far behind on everything and it's hard to concentrate and yeah the meeting with the boss that's coming up is not <5:20:52 PM> Anthony - patient: probably going not going to go too well <5:20:55 PM> Anthony - patient: it has been <5:20:56 PM> Bethany - doctor: it seems like it's been like really challenging for you <5:21:00 PM> Bethany - doctor: right yeah <5:21:03 PM> Anthony - patient: thank you <5:21:04 PM> Bethany - doctor: has like the energy and the focus affected other areas of your life <5:21:05 PM> Anthony - patient: so <5:21:05 PM> Anthony - patient: yeah at home my roommate just moved out so I come home I'm just alone it's just me the apartment <5:21:17 PM> Bethany - doctor: yeah why did your roommate move out <5:21:21 PM> Anthony - patient: is definitely been a big change and doesn't help the situation <5:21:26 PM> Anthony - patient: he just he moved he had to move away from work we were pretty close we were friends we would do things together and now that he's gone I haven't been doing too much after work <5:21:29 PM> Bethany - doctor: yeah <5:21:40 PM> Anthony - patient: you know socialization <5:21:42 PM> Bethany - doctor: do you have other friends in the area <5:21:43 PM> Anthony - patient: yeah um know my family lives across the state and I know a couple people from work but then once I see them at work it's not we never meet up afterwards <5:21:45 PM> Bethany - doctor: it sounds pretty lonely <5:21:58 PM> Anthony - patient: yeah yeah husband <5:22:02 PM> Bethany - doctor: what are you doing to occupy your time then <5:22:03 PM> Anthony - patient: um <5:22:08 PM> Anthony - patient: not so much I wish I could get <5:22:11 PM> Anthony - patient: your meet new people were like I used to exercise I haven't been exercising much <5:22:14 PM> Bethany - doctor: hmm what have you been doing for self-care <5:22:16 PM> Anthony - patient: so I haven't done anything <5:22:19 PM> Anthony - patient: um <5:22:25 PM> Anthony - patient: not much I've been trying to eat healthier you know I've been trying to eat some vegetables and lay off all the junk food but that can be a struggle sometimes too just busy at work and I don't have enough time to grocery stop and I'm trying to get caught up on everything that up behind <5:22:37 PM> Bethany - doctor: you know <5:22:44 PM> Anthony - patient: yeah <5:22:45 PM> Bethany - doctor: sounds pretty overwhelming <5:22:46 PM> Anthony - patient: it is <5:22:47 PM> Bethany - doctor: and with your roommate moving out are you able to afford rent on your own <5:22:49 PM> Anthony - patient: yeah yeah like I still have my <5:22:58 PM> Anthony - patient: decent income now with the job but we'll see how that goes <5:22:58 PM> Anthony - patient: Its <5:23:01 PM> Bethany - doctor: do you have concerns that you might lose your job <5:23:03 PM> Anthony - patient: performance is slacking <5:23:05 PM> Anthony - patient: yes yes that is my biggest concern right now <5:23:13 PM> Anthony - patient: I like like you mentioned I need this to pay rent in the roommates gone I need to step things up <5:23:15 PM> Bethany - doctor: having <5:23:15 PM> Bethany - doctor: yeah I can understand that some pretty big concern <5:23:22 PM> Anthony - patient: become more efficient be able to focus a little more <5:23:26 PM> Anthony - patient: yes <5:23:30 PM> Bethany - doctor: been able to talk to your boss at all about kind of what's going on if you feel comfortable <5:23:36 PM> Anthony - patient: now I haven't talked to him I'm a little afraid to and he scheduled a meeting that's coming up this week and that'll be the first time I've talked to him for a while <5:23:36 PM> Bethany - doctor: yeah <5:23:46 PM> Anthony - patient: I'm not sure I'm not sure like if I can be honest with him and sort of this depression that I've been going through I want to be honest but it's not really an environment where that not sure how they'll react to that <5:23:46 PM> Bethany - doctor: of what do you plan on saying in the meeting <5:23:50 PM> Bethany - doctor: hmm <5:24:07 PM> Bethany - doctor: okay um <5:24:11 PM> Bethany - doctor: what coping strategies do you have with everything that's been going on <5:24:14 PM> Anthony - patient: um I sleep a lot whenever I get overwhelmed or too down like I just go to bed early I sleep in like on the weekends I sort of sleep all day I try to get outside when it's nice and walk my neighborhood <5:24:23 PM> Bethany - doctor: and how does sleeping a lot make you feel <5:24:30 PM> Anthony - patient: I would say that's about my coping mechanism <5:24:34 PM> Anthony - patient: even more tired <5:24:36 PM> Bethany - doctor: okay <5:24:39 PM> Anthony - patient: it's not it's a lot of sleep but not great quality sleep <5:24:40 PM> Bethany - doctor: and what are your thoughts about following up with like mental health counseling maybe making appointment with them <5:24:50 PM> Anthony - patient: yeah I know you mentioned that last time and I want to I just haven't gotten to it yet <5:24:51 PM> Bethany - doctor: yeah what do you think is like the biggest obstacle for getting an appointment set up with them <5:24:58 PM> Anthony - patient: um <5:25:07 PM> Anthony - patient: pick up <5:25:07 PM> Anthony - patient: finding the time to just <5:25:10 PM> Anthony - patient: the phone and call I have the time but when I get home from work I'm just so tired <5:25:16 PM> Bethany - doctor: yeah <5:25:18 PM> Bethany - doctor: I understand it can be like challenging especially with everything you're going through have you considered though that you know it might be worth your time to <5:25:30 PM> Bethany - doctor: see them <5:25:31 PM> Bethany - doctor: yeah if you like do make an appointment go see them like what do you what do you think like benefits you would get from an appointment like that <5:25:32 PM> Anthony - patient: yeah um <5:25:34 PM> Anthony - patient: now that we're talking about it more yeah like I think I realized that I need to do it but I just haven't gotten there <5:25:46 PM> Anthony - patient: um ideally I think that would help me sort of <5:25:52 PM> Bethany - doctor: yeah <5:25:57 PM> Anthony - patient: option with like the interpret the presence that we're refilling today I think <5:26:02 PM> Anthony - patient: sort of giving me Copa mechanisms and maybe <5:26:06 PM> Anthony - patient: sort of like you said methods of <5:26:09 PM> Anthony - patient: maybe finding socialization or trying to exercise more get more active and then maybe all my energy levels will rise more and I'll be able to focus <5:26:18 PM> Anthony - patient: he can be <5:26:21 PM> Bethany - doctor: I agree I think that mental health counseling could be beneficial in creating some coping mechanisms to try and help you when things get difficult like this upcoming meeting with your boss and losing your roommate and feeling lonely I think it could be very much so worth your time to talk to someone <5:26:33 PM> Anthony - patient: yeah I agree with you <5:26:42 PM> Bethany - doctor: okay and on a scale of one to ten How likely are you to make an appointment with them coming up here <5:26:44 PM> Anthony - patient: it um <5:26:51 PM> Bethany - doctor: okay <5:26:53 PM> Anthony - patient: I think it's been like two out of 10 I just haven't been motivated to but I think talking with you now I know it's important so maybe a seven out of 10 <5:26:59 PM> Bethany -
