patient: and I think the one for the diabetes if I can remember but <6:13:03 PM> Bethany - patient: it's it's it's it's too many <6:13:06 PM> Bethany - patient: well I just I don't see why I have to say I'm already on all these other medications <6:13:08 PM> Anthony - doctor: can you tell me why you aren't taking the other ones <6:13:17 PM> Anthony - doctor: okay why do you think I provided you with the other ones <6:13:17 PM> Bethany - patient: well probably because I'm sick I'm just you know I'm old and dying over here so <6:13:23 PM> Anthony - doctor: and how do you feel about that <6:13:28 PM> Bethany - patient: oh well I've had a long life so <6:13:34 PM> Anthony - doctor: Okay so <6:13:36 PM> Bethany - patient: I'm just you know <6:13:38 PM> Bethany - patient: home and don't really do much but that's that's okay <6:13:46 PM> Anthony - doctor: you're okay with being sick and old and dying <6:13:49 PM> Bethany - patient: well I mean no one's okay with being sick and old and dying but fact of life <6:13:51 PM> Anthony - doctor: okay and what what are you hoping to get out of <6:13:51 PM> Anthony - doctor: this <6:14:00 PM> Anthony - doctor: I meant today if you don't want more meds <6:14:03 PM> Bethany - patient: I just want to make sure that I'm not gonna die tomorrow <6:14:03 PM> Anthony - doctor: okay so you're concerned about not dying tomorrow <6:14:06 PM> Bethany - patient: mm-hmm <6:14:11 PM> Anthony - doctor: what about in two days <6:14:12 PM> Bethany - patient: well I mean hopefully not them either but <6:14:20 PM> Bethany - patient: well I just <6:14:21 PM> Anthony - doctor: how are we going to prevent you how can we help you not die tomorrow where the day after <6:14:27 PM> Anthony - doctor: why do we want why are you concerned about your Labs or getting your blood pressure taken <6:14:28 PM> Bethany - patient: got to make sure that you know there's nothing hiding you know got to get my labs done and have my blood pressure taken but <6:14:36 PM> Bethany - patient: make sure they're you know <6:14:42 PM> Anthony - doctor: okay and how do you think <6:14:45 PM> Bethany - patient: doing all right <6:14:51 PM> Bethany - patient: I've been taking my blood pressure medication it's the one I do take <6:14:52 PM> Anthony - doctor: will be today <6:14:53 PM> Anthony - doctor: without taking medications <6:15:00 PM> Bethany - patient: but yeah the other ones I don't know you know <6:15:01 PM> Anthony - doctor: okay how did how do you use your tell me they don't make you feel great how do they make you <6:15:05 PM> Bethany - patient: so these other ones you know they don't make me feel feel great <6:15:12 PM> Bethany - patient: mm-hmm <6:15:16 PM> Bethany - patient: well the metformin gives me diarrhea for my <6:15:17 PM> Anthony - doctor: taking <6:15:17 PM> Anthony - doctor: okay so you don't want to take the metformin we have other options would you be open to <6:15:21 PM> Bethany - patient: my diabetes and you know sometimes I take it but it makes me you know feel so bad that I'm like I don't want to keep doing that <6:15:37 PM> Bethany - patient: well we got to figure out some of these other medications first because I'm on too many pills <6:15:40 PM> Anthony - doctor: other medications for your diabetes <6:15:43 PM> Anthony - doctor: okay so besides maybe some of them aren't making feeling good what's preventing you are there any other barriers to preventing or barriers taking them <6:15:48 PM> Bethany - patient: that's why I want my labs done so I can see how everything's doing and then those are good I don't want to keep taking these medications <6:15:57 PM> Bethany - patient: yeah so one of them I'm supposed to take three times a day another one it says at night the other one says in the morning with breakfast but I don't even eat breakfast so there's just so much to figure out with each of them that makes it challenging to be able to take them all every day <6:16:22 PM> Anthony - doctor: here and it is a lot and it's a lot of pills to swallow different times <6:16:26 PM> Bethany - patient: well I have a box with all of them <6:16:32 PM> Anthony - doctor: have we tried are you opening or have you tried any tools to keep track of when and how to take each of them <6:16:42 PM> Anthony - doctor: okay and <6:16:45 PM> Bethany - patient: yeah I mean but <6:16:50 PM> Anthony - doctor: does that help <6:16:54 PM> Bethany - patient: it helps keep them all together so I know where they're at but I got one of those pill organizers but it's only a.m. and p.m. and what I'm supposed to take three times a day where am I supposed to put the third one <6:17:00 PM> Anthony - doctor: I hear that um maybe we can look into getting you a better organizer is <6:17:06 PM> Bethany - patient: well maybe if we could condense them who changed at all as far as like like when I'm supposed to take them so it makes sense that could help because I don't eat breakfast I don't know if I'm supposed to take it at lunch or not <6:17:12 PM> Anthony - doctor: what help what do you see what how with keeping track of the when and how you're supposed to take your pills <6:17:33 PM> Anthony - doctor: okay and yeah let's maybe we can find some mobile apps or on your calendar make a list of when we're supposed to take them but those are <6:17:35 PM> Bethany - patient: I mean after that <6:17:46 PM> Bethany - patient: they make an app for that <6:17:47 PM> Anthony - doctor: sorry you cut out <6:17:51 PM> Anthony - doctor: yeah sure we can maybe later today after the appointment we can walk you through <6:17:55 PM> Bethany - patient: okay will it alert me when I'm supposed to take each pill <6:17:57 PM> Anthony - doctor: yeah <6:18:00 PM> Bethany - patient: oh <6:18:01 PM> Anthony - doctor: okay so what other barriers if that's the only barrier <6:18:02 PM> Bethany - patient: yeah well that was a big one is I didn't like how that one made me feel and the one that don't even eat when I'm supposed to take it but the other thing is sometimes I like well nap during the day I stay at home and retired so I just you know sat on the couch watch my watch my shows and sometimes I'll take a nap and then I'll sleep through when I'm supposed to take my pill <6:18:08 PM> Anthony - doctor: what other barriers are preventing you from taking them we can talk about the metformin side effects if there's another drug but <6:18:37 PM> Bethany - patient: know if I should still take it or not because they'll be too close to my next pill <6:18:40 PM> Anthony - doctor: will <6:18:43 PM> Anthony - doctor: I <6:18:43 PM> Anthony - doctor: okay <6:18:46 PM> Anthony - doctor: again these are challenging you have <6:18:49 PM> Anthony - doctor: Vodacom where Betty's a lot of things going on and this challenging maybe that app will we can set alarms that will go off and wake you up <6:18:58 PM> Bethany - patient: well doc I mean we're all gonna die but first of all if we can just get these you know to where it's like reasonable for you to take all these then that'll help we can start there <6:18:58 PM> Anthony - doctor: but at the beginning of this conversation you mentioned that your old <6:19:04 PM> Anthony - doctor: and it's just <6:19:05 PM> Anthony - doctor: you're maybe die soon but you don't want to die today <6:19:10 PM> Anthony - doctor: how do you feel at this point if we're able to straighten out the medications <6:19:15 PM> Anthony - doctor: how do you feel about that <6:19:22 PM> Anthony - doctor: okay <6:19:33 PM> Anthony - doctor: so <6:19:37 PM> Anthony - doctor: yeah ________________________________________________________________________________ End of transcript at Aug 25, 2025, 6:19:43 PM
