Patient: yeah I just need to refill my medications <12:49:07 PM> Lily Chen - Doctor: okay okay um so kind of to review what we did last time looks like the antidepressant to you started wasn't working quite well you know for you at the lower dose so we increased it <12:49:13 PM> Alina - Patient: that's okay <12:49:26 PM> Alina - Patient: um <12:49:29 PM> Lily Chen - Doctor: how have you been feeling since then <12:49:32 PM> Lily Chen - Doctor: okay um if you're okay sharing with me please tell me a bit more about some of the things that are stressors in your life right now <12:49:34 PM> Alina - Patient: maybe a little better not like pretty similar I guess maybe a little better <12:49:41 PM> Alina - Patient: feel like it's just like I've had some like stress so you know it's hard to gauge <12:49:47 PM> Alina - Patient: um well my roommate moved out so <12:49:57 PM> Lily Chen - Doctor: mm-hmm <12:50:00 PM> Alina - Patient: kind of added to my stress just having you know like an increased rent payment but <12:50:06 PM> Alina - Patient: we're working on it <12:50:08 PM> Lily Chen - Doctor: um <12:50:08 PM> Lily Chen - Doctor: okay okay <12:50:11 PM> Lily Chen - Doctor: room anything else that you wanted to talk about today <12:50:14 PM> Alina - Patient: um no not really just feel like you know I just need to keep taking my antidepressant and maybe it'll help <12:50:16 PM> Lily Chen - Doctor: the other stressors or <12:50:18 PM> Lily Chen - Doctor: okay okay um how have you been feeling on the antidepressant in terms of like how much it's helped your mood <12:50:32 PM> Alina - Patient: um I feel like maybe I'm a little less like irritable than before <12:50:36 PM> Lily Chen - Doctor: okay <12:50:42 PM> Alina - Patient: but <12:50:45 PM> Alina - Patient: you know <12:50:47 PM> Alina - Patient: other than that it's probably pretty similar <12:50:52 PM> Alina - Patient: I don't really think about it too much on a day-to-day though so I'm not really sure <12:50:53 PM> Lily Chen - Doctor: okay um <12:50:56 PM> Alina - Patient: um <12:50:59 PM> Lily Chen - Doctor: and just because I want to double check with everyone who's on an antidepressant any side effects with it <12:51:08 PM> Lily Chen - Doctor: okay and are you having any trouble taking it every day as prescribed or getting it from the pharmacy <12:51:10 PM> Alina - Patient: let me think <12:51:14 PM> Alina - Patient: I don't think so not that I really noticed <12:51:18 PM> Alina - Patient: um no not really you know sometimes I take it in the morning sometimes I take it at night if I you know Forget before work or something but I pretty good about taking it every day <12:51:31 PM> Lily Chen - Doctor: okay sounds good so sounds like you're taking it exactly as you're supposed to and helping a little bit but not quite where <12:51:38 PM> Alina - Patient: yeah I feel like you know just a little less irritable but I still have trouble like at work <12:51:49 PM> Lily Chen - Doctor: quite where you feel much better <12:51:53 PM> Lily Chen - Doctor: okay <12:52:00 PM> Alina - Patient: you know feeling like I can concentrate having the energy I need to <12:52:01 PM> Lily Chen - Doctor: okay okay um and then again just some of the questions we've asked before just want to ask out to everyone who comes into talk about their mood <12:52:05 PM> Alina - Patient: through the day doing my job well <12:52:09 PM> Alina - Patient: it's <12:52:18 PM> Alina - Patient: um <12:52:22 PM> Lily Chen - Doctor: still feeling like low mood <12:52:25 PM> Lily Chen - Doctor: okay um and you mentioned difficulty concentrating and a lot of stressors in your life <12:52:26 PM> Alina - Patient: yeah probably just a little like down sluggish feeling <12:52:41 PM> Lily Chen - Doctor: any like feeling like you're just not interested in activities you normally enjoy <12:52:48 PM> Alina - Patient: um <12:52:49 PM> Lily Chen - Doctor: all right <12:52:50 PM> Alina - Patient: maybe I just feel like I'm so busy I haven't really like noticed but probably I feel like you know when I come home from work like <12:52:59 PM> Alina - Patient: I just want to like you know lay around and <12:53:03 PM> Alina - Patient: I go to bed so I can have the energy hopefully have the energy I need for the next day <12:53:05 PM> Lily Chen - Doctor: yeah yeah it sounds like with all the stress that's going on your life it's very difficult and very tiring <12:53:17 PM> Lily Chen - Doctor: and then any thoughts of hurting yourself <12:53:18 PM> Alina - Patient: know <12:53:21 PM> Lily Chen - Doctor: okay um and then any <12:53:22 PM> Alina - Patient: you know <12:53:25 PM> Lily Chen - Doctor: any attempts to hurt yourself or anyone else since we last spoke okay <12:53:31 PM> Lily Chen - Doctor: um then we'll refill your medication at the end of the visit but if it's okay with you I'd also like to talk a little bit about the referral we sent last time for like the the behavioral consultant <12:53:31 PM> Alina - Patient: okay <12:53:49 PM> Lily Chen - Doctor: okay um what what happened regarding that <12:53:50 PM> Alina - Patient: um nothing really happened I haven't gone and seen them or anything haven't made an appointment so kind of just where we were last time <12:53:55 PM> Lily Chen - Doctor: okay okay could you tell me a bit more about like if you had any difficulties with that or what are your thoughts about that <12:54:12 PM> Lily Chen - Doctor: mm-hmm okay <12:54:12 PM> Alina - Patient: I just like haven't really found the time to call and schedule <12:54:19 PM> Alina - Patient: specific <12:54:21 PM> Lily Chen - Doctor: then on the <12:54:24 PM> Lily Chen - Doctor: so it sounds like um are you are you interested still in going to see them <12:54:31 PM> Alina - Patient: yeah I don't know like if that would maybe like you know add to my stress like having another thing to do on my list <12:54:40 PM> Alina - Patient: but you know I mean it seems like you know maybe you think it would help since you've brought it up the past few times so <12:54:43 PM> Lily Chen - Doctor: mm-hmm <12:54:49 PM> Alina - Patient: I guess I'm not like not interested but I'm also just kind of worried in my you know to my <12:54:56 PM> Alina - Patient: like weekly schedule <12:54:57 PM> Lily Chen - Doctor: yeah okay so sounds like there's kind of like a concern with a Time component here and that's kind of like the main barrier I guess to you feeling like you're ready to go um you mentioned that you think it could help what are what are some things that you think it could help with <12:55:03 PM> Alina - Patient: um maybe just like <12:55:18 PM> Lily Chen - Doctor: yeah <12:55:21 PM> Alina - Patient: having like alternative ways of dealing with my stress <12:55:27 PM> Alina - Patient: like coping mechanisms I guess and then you know might just help to have somebody to <12:55:32 PM> Alina - Patient: talk to about you know all my stressors <12:55:36 PM> Alina - Patient: I don't feel like I'm bothering people like my friends and family <12:55:37 PM> Lily Chen - Doctor: yeah um for sure it's um like you said it's helpful to have a third party <12:55:48 PM> Lily Chen - Doctor: and if you do have friends and family that you can talk to having someone else's <12:55:53 PM> Lily Chen - Doctor: I've heard a lot of patients also say similarly that it's very helpful to have that kind of more impartial person <12:56:03 PM> Lily Chen - Doctor: so it sounds like there's some there's some hesitancy but you you sound like you also have some idea of things that you could gain from going so given all that if I give you a scale from 0 to 10 with 0 being like I'm not ready at all to go and 10 being like I'm going to make the appointment today where would you put yourself on that scale <12:56:04 PM> Alina - Patient: he <12:56:28 PM> Lily Chen - Doctor: a five okay <12:56:28 PM> Lily Chen - Doctor: okay <12:56:30 PM> Alina - Patient: maybe like a five <12:56:35 PM> Alina - Patient: yeah <12:56:38 PM> Lily Chen - Doctor: so <12:56:38 PM> Alina - Patient: yeah <12:56:38 PM> Alina - Patient: yeah <12:56:39 PM> Lily Chen - Doctor: um maybe thinking about it but not quite sure yet <12:56:44 PM> Lily Chen - Doctor: right okay um yeah that's totally fair and <12:56:47 PM> Alina - Patient: I think it just <12:56:49 PM> Alina - Patient: I don't know it feels like a stressful task trying to find the right <12:56:53 PM> Alina - Patient: counselor <12:57:01 PM> Lily Chen - Doctor: um and I <12:57:01 PM> Paul Hershberger: I'll go ahead and stop your hair lowly <12:57:05 PM> Lily Chen - Doctor: um I hear you on that <12:57:07 PM> Lily Chen - Doctor: okay thanks <12:57:13 PM> Lily Chen - Doctor: he's sitting kind of funny in my legs going kind of numb <12:57:14 PM> Alina - Patient: I know I was I was shifting around because I was sitting on my foot <12:57:21 PM> Alina - Patient: it's so good it's a fallen asleep on me <12:57:23 PM> Paul Hershberger: the <12:57:23 PM> Lily Chen - Doctor: yeah ________________________________________________________________________________ End of transcript at Oct 14, 2025, 12:57:28 PM
