Patient: yes I am <6:08:21 PM> Isaac Stokes - Doctor: hello are you Rachel DeSoto <6:08:26 PM> Isaac Stokes - Doctor: all right hey Rachel my name is Isaac I'm a third year medical student I'm the doctors excuse me on your team here today looks like you are in to talk about your antidepressant refills today is that correct <6:08:31 PM> Rachel - Patient: but yeah that's why I made the appointment today <6:08:44 PM> Isaac Stokes - Doctor: okay well what can you tell me just a little bit about how everything's been going on your end <6:08:48 PM> Rachel - Patient: um yeah I <6:08:57 PM> Rachel - Patient: I got the prescription for like my antidepressant like increased <6:09:04 PM> Rachel - Patient: it's kind of been working not I mean it's been like okay um just kind of wanted to come in and talk about it I don't know if it's working that well but <6:09:12 PM> Isaac Stokes - Doctor: and how how frequently have you been taking this medication <6:09:17 PM> Rachel - Patient: I've been all right <6:09:23 PM> Rachel - Patient: um I take it like every day <6:09:26 PM> Isaac Stokes - Doctor: okay and how is your mood been <6:09:26 PM> Rachel - Patient: um I mean it's been like fine I guess <6:09:30 PM> Isaac Stokes - Doctor: it's <6:09:35 PM> Rachel - Patient: don't know <6:09:36 PM> Isaac Stokes - Doctor: well and I know that we increase the dose about three months ago have you noticed much of a difference in that time as far as your mood is concerned <6:09:36 PM> Rachel - Patient: um I mean I feel like I'm like less depressed sometimes I guess but not like <6:09:50 PM> Isaac Stokes - Doctor: okay one so I'm hearing that you're still having some difficulties in that day-to-day and just struggling throughout that is that correct <6:09:55 PM> Rachel - Patient: super happy I I suppose that's what I mean <6:10:00 PM> Rachel - Patient: yeah I mean like I still have a lot of things I'm like stressed about and like worried about I guess so <6:10:09 PM> Isaac Stokes - Doctor: tell me more about that <6:10:17 PM> Rachel - Patient: yeah so like <6:10:18 PM> Isaac Stokes - Doctor: so you <6:10:21 PM> Rachel - Patient: and I'm I'm worried about like <6:10:22 PM> Isaac Stokes - Doctor: just <6:10:24 PM> Rachel - Patient: um I like a job interview coming up and like I'm pretty worried about that and like <6:10:31 PM> Rachel - Patient: don't know <6:10:33 PM> Rachel - Patient: how to deal with that and just like some other things going on and <6:10:37 PM> Isaac Stokes - Doctor: I see that definitely sounds like there's quite a bit on your plate especially with that job interview just trying to balance everything on your end thank you very much for coming back into the office for this appointment it's it is very good to see and I'm happy that we have the chance to talk face-to-face like this now I know that we were also in the past talking about getting in touch with that mental health counselor where we in regards to that <6:10:48 PM> Rachel - Patient: yeah I just like haven't been able to make the appointment I think <6:11:18 PM> Isaac Stokes - Doctor: how come <6:11:20 PM> Rachel - Patient: you know they said something about like reaching out to me or maybe I was supposed to call them I don't know I just didn't end up doing it <6:11:27 PM> Rachel - Patient: yeah I just I guess I was you know I don't I just didn't get around to it I guess I was just <6:11:38 PM> Isaac Stokes - Doctor: now is this <6:11:39 PM> Rachel - Patient: room <6:11:41 PM> Rachel - Patient: Seattle <6:11:43 PM> Rachel - Patient: so I don't know <6:11:44 PM> Rachel - Patient: um <6:11:44 PM> Isaac Stokes - Doctor: okay I got you is this a resource that you were still interested in pursuing further <6:11:54 PM> Rachel - Patient: yeah I mean I think it could be helpful <6:11:59 PM> Rachel - Patient: like I think if they you know kind of talking about <6:12:04 PM> Rachel - Patient: maybe like <6:12:05 PM> Rachel - Patient: how to cope with some of these things I guess I don't know <6:12:09 PM> Isaac Stokes - Doctor: what are your hesitations with it <6:12:10 PM> Rachel - Patient: I don't know how much help they would be but <6:12:13 PM> Rachel - Patient: um I guess like just getting everything set up it just seems like it could be a lot and it's like talking to another person about everything and <6:12:28 PM> Rachel - Patient: and <6:12:28 PM> Rachel - Patient: out seems like a whole thing <6:12:31 PM> Rachel - Patient: just kind of all like who said to jump through and worrying about I don't know like paying for and all that kind of thing <6:12:36 PM> Isaac Stokes - Doctor: and I think those are all very reasonable concerns to have moving forward with this regarding the actual schedule scheduling of it that is something that we could help out on our end we can place that referral once again and even place a few calls to make sure that they're contacting you in a timely manner and then we can even follow up with you in a couple weeks to make sure that is something that is getting scheduled and is a good fit for you <6:12:40 PM> Rachel - Patient: yeah <6:12:45 PM> Rachel - Patient: yeah that would that would be good if they like <6:12:45 PM> Rachel - Patient: called me <6:13:12 PM> Isaac Stokes - Doctor: how does this sound to you <6:13:19 PM> Rachel - Patient: and you guys kind of got a referral in so I could kind of <6:13:22 PM> Isaac Stokes - Doctor: now I'm I was wondering what your level of familiarity was with <6:13:24 PM> Rachel - Patient: you know make sure that like got followed up with <6:13:29 PM> Rachel - Patient: I mean I guess I'm not like super familiar I mean I know <6:13:36 PM> Isaac Stokes - Doctor: being both on the antidepressant and also going to a mental health counselor and how those treatment options interact with each other <6:13:55 PM> Rachel - Patient: I figure those would probably work better but I guess not like no one's ever <6:14:00 PM> Isaac Stokes - Doctor: would that be something that you'd be comfortable with me explaining a little bit more about during this appointment <6:14:01 PM> Rachel - Patient: explicitly talked to me about it <6:14:10 PM> Rachel - Patient: yeah sure <6:14:10 PM> Isaac Stokes - Doctor: yeah yeah so one thing that we see in a lot of our patients is <6:14:12 PM> Rachel - Patient: yeah I mean that makes sense yeah <6:14:18 PM> Isaac Stokes - Doctor: patients who take an antidepressant they're usually able to find some Solace with that medication and get back on their feet but when it is paired with also going to a counseling service that exponentially boosts the ability for our patients to get over those humps in their life and get over those tougher times in those tougher situations so that's typically why we like to do the antidepressant in coordination with the counseling services because it gives that extra boost to the treatment options with that so when I'm recommending that we also get the mental health counselor on board it's because we had seen some really positive outcomes and some of our other patient populations <6:15:12 PM> Isaac Stokes - Doctor: here now on a scale from 0 to 10 zero being <6:15:18 PM> Rachel - Patient: you know <6:15:18 PM> Rachel - Patient: yeah I mean I'd say like <6:15:23 PM> Isaac Stokes - Doctor: don't want to access the health counselor whatsoever 10 being You're Gonna Go assess the health counselor tomorrow what is how would you rank your future progression with <6:15:44 PM> Rachel - Patient: like a seven I guess <6:15:44 PM> Isaac Stokes - Doctor: okay that's that's great to hear I can definitely see that you're pretty motivated to actually go and get this scheduled and once again that is something that I would be more than happy to help out with on my end um one thing that is important to do and to continue doing is just making sure that we are adhering to the antidepressant regiment but once again if you have any questions please feel free to contact our office ________________________________________________________________________________ End of transcript at Nov 25, 2025, 6:16:20 PM
