Patient: six different medications I have to <6:49:57 PM> Joseph - Patient: you take them at <6:49:59 PM> Joseph - Patient: in the day in the afternoon and then at night and then you know with all those medications I'm missing I'm sometimes missing to take some and at certain points I just I don't care if I take them anymore it just be easier if I stop taking them <6:50:09 PM> Kate-Doctor: okay so I'm hearing that you feel like you are taking them too <6:50:16 PM> Joseph - Patient: look <6:50:25 PM> Joseph - Patient: a lot of pills and it's a lot of pills to have to take just a lot of times during the day yeah <6:50:26 PM> Kate-Doctor: okay so do you think that if we were able to work it out so that you only took medications once or twice a day would that be helpful for you <6:50:43 PM> Joseph - Patient: kind of <6:50:43 PM> Joseph - Patient: it certainly helped but it's just you know it's <6:50:48 PM> Joseph - Patient: like what point is too many medications I'm already on <6:50:54 PM> Joseph - Patient: I have so many things with me diabetes my blood pressure my kidneys and it's like <6:50:56 PM> Kate-Doctor: mm-hmm <6:50:59 PM> Kate-Doctor: sure yeah another thing you had mentioned is like would it be better to just not be on them at all right so we can talk about the the lab values and the things we see in the clinic that tell us the medications are helping you but I mean how you feel about the overall Improvement is important too right and the thing with diabetes and hypertension high blood pressure is that a lot of the things that we do in just our everyday life has a big impact on our health so what do you think about how your lifestyle of impacts your <6:51:03 PM> Joseph - Patient: at a certain point what one do the medication stop working <6:51:38 PM> Joseph - Patient: how we leave I know we've talked about it a lot trying to eat healthy or get active but I just honestly I just haven't been doing it I just find myself not <6:51:49 PM> Kate-Doctor: health <6:51:51 PM> Kate-Doctor: here <6:51:56 PM> Kate-Doctor: okay what barriers do you see to that what obstacles are there for you <6:52:02 PM> Joseph - Patient: are wanting to <6:52:04 PM> Joseph - Patient: it I think part of it is just trying it <6:52:15 PM> Joseph - Patient: difficult for me to <6:52:18 PM> Joseph - Patient: eat the foods that I should be eating it's much easier you know to stay with my what I normally eat I don't want to change and then with <6:52:19 PM> Kate-Doctor: yeah the cold is certainly a barrier to exercising outside do you feel like there's anything inside that you could be doing <6:52:29 PM> Joseph - Patient: us trying to stay active it's just much easier to stay inside take it easy <6:52:36 PM> Joseph - Patient: not get outside in the cold and just easier to be <6:52:41 PM> Joseph - Patient: keep to my usual self <6:52:50 PM> Joseph - Patient: as I'm sure there is I I could <6:53:00 PM> Joseph - Patient: what yeah I could start trying to do some exercises inside <6:53:01 PM> Kate-Doctor: okay so let me ask you this if you don't make any changes to your lifestyle <6:53:10 PM> Joseph - Patient: I'm already <6:53:12 PM> Kate-Doctor: heard how do you see that affecting you 10 years from now <6:53:16 PM> Kate-Doctor: five years from now <6:53:19 PM> Joseph - Patient: I'm already up there doc yeah I I don't see <6:53:24 PM> Joseph - Patient: just ends with me most likely dying I don't see <6:53:29 PM> Joseph - Patient: what difference does those last few years make <6:53:29 PM> Kate-Doctor: here do you have things about your life that you enjoy that you would like to continue to experience <6:53:37 PM> Joseph - Patient: yeah well I mean I would you know I'd like to see my grandkids grow up I'd like I'd like to be a part of their life but I'm just worried that there's already <6:53:46 PM> Kate-Doctor: oh I see okay <6:53:53 PM> Joseph - Patient: too much wrong with me <6:53:55 PM> Joseph - Patient: you know <6:53:55 PM> Joseph - Patient: here right now it's just easy I don't <6:53:59 PM> Kate-Doctor: so <6:54:04 PM> Kate-Doctor: so so what do you like about your current lifestyle <6:54:12 PM> Kate-Doctor: okay it's easy sure <6:54:14 PM> Joseph - Patient: would eat my routine <6:54:16 PM> Kate-Doctor: yeah it would be easier sure but um what don't you like about it <6:54:18 PM> Joseph - Patient: a lot more comfortable minus all the medications I have to take but it's it's much more easier for me to just not make any change <6:54:29 PM> Joseph - Patient: okay well I I can definitely tell I'm get I'm getting worse I'm losing my breath more <6:54:35 PM> Kate-Doctor: she never <6:54:42 PM> Joseph - Patient: just difficulties getting moving around I'm not able to play with my grandkids as much as I was <6:54:49 PM> Joseph - Patient: need 10 5 10 years ago so yeah it's impacting me in that way <6:54:56 PM> Joseph - Patient: you know I I think it would be I'd like to be I mean I'd like to be around for as long as I can just for them <6:54:56 PM> Kate-Doctor: okay so if we were able to make it easier for you to get up and interact with those grandkids do you think that would enrich those last the next several years of your life <6:55:11 PM> Kate-Doctor: yeah and and it's not just about extending your life right like we want to increase the quality of your life as well like you said you want to be able to get up and move and interact with those grandkids and I mean they're difficult to keep up with for sure <6:55:18 PM> Joseph - Patient: yeah <6:55:35 PM> Kate-Doctor: so what are some changes that you think you would be willing to make to to set yourself on that path <6:55:39 PM> Joseph - Patient: well you talked about it earlier I'd like to <6:55:45 PM> Joseph - Patient: I'd like to <6:55:47 PM> Joseph - Patient: at least consolidate some of the medications make it easier so I don't miss doses <6:55:54 PM> Joseph - Patient: I guess I could try and we talked about it earlier try to be more active <6:55:59 PM> Joseph - Patient: in the house in the colder weathers Maybe <6:56:04 PM> Joseph - Patient: just work out <6:56:07 PM> Joseph - Patient: even <6:56:07 PM> Joseph - Patient: by myself in the house maybe walk around more <6:56:12 PM> Joseph - Patient: playing with the grandkids might keep me more active as well <6:56:17 PM> Kate-Doctor: sure and what about your <6:56:23 PM> Joseph - Patient: but we talked about a lot I could have I could <6:56:25 PM> Kate-Doctor: it <6:56:26 PM> Kate-Doctor: and you know every Small Change adds up so you could even just start with a small goal and just add to it as you feel comfortable on a scale of one to ten how motivated would you say you are to maybe make some changes to your <6:56:28 PM> Joseph - Patient: I know what foods I should be eating it's just <6:56:28 PM> Joseph - Patient: part <6:56:32 PM> Joseph - Patient: it's getting them having to go out by the foods that I should be eating <6:56:39 PM> Joseph - Patient: make those changes but I could try and at least cut back on a little bit <6:56:52 PM> Joseph - Patient: to the diet maybe maybe like a four <6:57:02 PM> Kate-Doctor: it <6:57:03 PM> Kate-Doctor: okay and then what about for exercising <6:57:06 PM> Joseph - Patient: the exercising I'd say six six for exercising <6:57:11 PM> Kate-Doctor: okay <6:57:15 PM> Kate-Doctor: I think that's a great start <6:57:15 PM> Joseph - Patient: oh well for the exercise I want to be active I want to play with my grandkids I definitely want to prove in that sense and then <6:57:18 PM> Kate-Doctor: so why not lower numbers <6:57:22 PM> Kate-Doctor: steps <6:57:31 PM> Joseph -
