Patient: here <2:58:02 PM> Ian Carmody - Patient: yeah it's been fine I just don't think the the meds are really doing anything <2:58:06 PM> Ananya - Doctor: oh okay why do you think that <2:58:09 PM> Ian Carmody - Patient: here um <2:58:11 PM> Ananya - Doctor: oh okay and from what I understand it looks like three months ago the dose was increased but you're still feeling this way even with that happening <2:58:13 PM> Ian Carmody - Patient: just kind of feel the same like I don't <2:58:17 PM> Ian Carmody - Patient: feel kind of down still <2:58:20 PM> Ian Carmody - Patient: yeah can we maybe just like try a different medication <2:58:29 PM> Ananya - Doctor: um well I just kind of want to ask you a few more questions to see more about you know how your mood has been in the interim and then we can maybe discuss some different things we could do and one could be either increasing your medication or switching but <2:58:29 PM> Ananya - Doctor: I want to <2:58:32 PM> Ian Carmody - Patient: it just don't feel any different than I did before I still feel kind of <2:58:50 PM> Ananya - Doctor: see why exactly you feel like the medication hasn't been working <2:58:58 PM> Ananya - Doctor: okay and do you feel like the medication has helped in any aspects with your mood <2:58:59 PM> Ian Carmody - Patient: sad and have not really that much energy to do things <2:59:07 PM> Ian Carmody - Patient: maybe a little bit maybe maybe feel a little more <2:59:10 PM> Ananya - Doctor: pressure <2:59:13 PM> Ananya - Doctor: okay and can you tell me more about any other life changes that have been happening <2:59:15 PM> Ian Carmody - Patient: able but <2:59:17 PM> Ian Carmody - Patient: doesn't really make me feel happy <2:59:19 PM> Ian Carmody - Patient: um well I have been really stressed out about work I've gotten kind of a bad review from my boss recently <2:59:25 PM> Ananya - Doctor: okay and how do you feel like your mood has been affecting your work and this eval <2:59:36 PM> Ian Carmody - Patient: I just haven't had the motivation to kind of pull through with a couple projects I was supposed to work on <2:59:42 PM> Ian Carmody - Patient: well I think it's a lot because <2:59:50 PM> Ananya - Doctor: okay <2:59:52 PM> Ian Carmody - Patient: I just don't really have the motivation or energy to do things at work because I'm kind of stuck in my own head thinking about things <2:59:52 PM> Ian Carmody - Patient: and <3:00:01 PM> Ian Carmody - Patient: it just doesn't seem that important <3:00:02 PM> Ananya - Doctor: okay <3:00:04 PM> Ian Carmody - Patient: well I was hoping the medication could do that <3:00:06 PM> Ananya - Doctor: you feel like you could do to maybe improve your concentration for work <3:00:13 PM> Ananya - Doctor: okay <3:00:19 PM> Ananya - Doctor: um <3:00:21 PM> Ananya - Doctor: it's horrible thankful to us <3:00:28 PM> Ananya - Doctor: when we lost or when we put you on the medication what was your understanding of how it would help your mood <3:00:31 PM> Ian Carmody - Patient: it um <3:00:31 PM> Ian Carmody - Patient: I <3:00:40 PM> Ian Carmody - Patient: so <3:00:42 PM> Ian Carmody - Patient: really know I was kind of like <3:00:45 PM> Ian Carmody - Patient: to depression right so it's kind of just supposed to make me not depressed kind of give me <3:00:51 PM> Ananya - Doctor: okay yeah <3:00:51 PM> Ian Carmody - Patient: boost in mood <3:00:53 PM> Ian Carmody - Patient: time <3:00:55 PM> Ananya - Doctor: um and <3:00:57 PM> Ananya - Doctor: if we think about maybe aside from medication used to help with your mood is there anything else you're able to do in your life to help maybe increase your mood <3:01:07 PM> Ananya - Doctor: okay yeah we did I did give you a referral for some mental health counseling so from what I'm hearing you didn't make an appointment with them <3:01:10 PM> Ian Carmody - Patient: I think like seeing people I I'm friends with but then my roommate moved out recently too so I'm a little bit more alone than usual I know you talked about therapy before but I just I don't know I don't think that works <3:01:28 PM> Ian Carmody - Patient: uh no <3:01:34 PM> Ananya - Doctor: okay is there any reason that you chose not to <3:01:36 PM> Ian Carmody - Patient: I just thought the medication would kind of figure it out and you know it takes a lot of time to go to this appointments and <3:01:36 PM> Ian Carmody - Patient: just <3:01:40 PM> Ananya - Doctor: okay um <3:01:48 PM> Ian Carmody - Patient: didn't feel like it <3:01:52 PM> Ananya - Doctor: if you were to do counseling what do you feel like they could help you with <3:01:54 PM> Ian Carmody - Patient: um well I guess they're kind of the experts on this thing so <3:01:58 PM> Ananya - Doctor: yeah I mean the decision is obviously going to be up to you but usually what we recommend is the combination of a medication and counseling just because they can help you with maybe talking through some of these concerns you mentioned such as your job in the evaluation coming up and also with your roommate moving out and feeling a little lonely they can kind of help you think of ways to increase your mood outside of medication so on a scale of zero to ten How likely now do you feel like you would be to make that appointment since I referred you <3:02:04 PM> Ian Carmody - Patient: maybe they would have some some tips for things I could do <3:02:10 PM> Ian Carmody - Patient: on that boost my mood <3:02:42 PM> Ian Carmody - Patient: um maybe like a five out of 10 <3:02:49 PM> Ananya - Doctor: okay any reason it's not lower or higher <3:02:53 PM> Ian Carmody - Patient: um <3:02:59 PM> Ian Carmody - Patient: I think it's higher than it used to be since the medication isn't working so I'm just starting to get a little more desperate <3:03:07 PM> Ian Carmody - Patient: Maybe <3:03:10 PM> Ian Carmody - Patient: maybe just I still have my doubts and maybe just like a little bit of <3:03:16 PM> Ian Carmody - Patient: time constraints that make me don't want to do it as much <3:03:19 PM> Ananya - Doctor: okay that's fair um and just thinking about maybe moving forward with counseling what change do or what would change look like for you how do you think <3:03:21 PM> Ian Carmody - Patient: mmm <3:03:33 PM> Ananya - Doctor: is what could you get out of counseling that would be useful for you <3:03:37 PM> Ananya - Doctor: yeah that would be really helpful <3:03:39 PM> Ian Carmody - Patient: maybe if they can help me like come up with strategies to kind of face some of my problems like when I'm feeling down <3:03:39 PM> Ian Carmody - Patient: how <3:03:47 PM> Ian Carmody - Patient: could help me get around those thoughts and maybe turn it into something productive and like getting getting back to and meeting new people ________________________________________________________________________________ End of transcript at Mar 9, 2026, 3:04:02 PM
