Patient: yeah it's just I mean I'm on like 20 different pills that I have to take some in the morning some in the afternoon some in the evening and you know I've been watching a lot of TV and I see these shows like like Dr Oz and he tells me that I can just eat some of this gelatin and then I don't have to take half these pills so I don't know I like our doctors just getting paid to give me these medications like you work with the pharmacy or <3:29:15 PM> Ian Carmody - Patient: why <3:29:47 PM> Ananya - Doctor: oh okay yeah um why don't you tell me a little bit what about what you know are the benefits of the medications that you're taking <3:29:53 PM> Ian Carmody - Patient: you why do I have to take all these <3:29:56 PM> Ian Carmody - Patient: um I mean <3:30:08 PM> Ian Carmody - Patient: I'm like heard that blood pressure can <3:30:13 PM> Ian Carmody - Patient: give your heart problems I know I have some heart problems I don't want to heart attack <3:30:13 PM> Ian Carmody - Patient: had heart problems before and I got a stent but why can't I just get another stent if I have another heart problem <3:30:35 PM> Ananya - Doctor: okay so it sounds like you just don't feel like you're actually seeing the benefits of these meds that you're taking <3:30:36 PM> Ian Carmody - Patient: mm-hmm <3:30:42 PM> Ananya - Doctor: um can you tell me about what the consequence consequences of your medications are if any <3:30:52 PM> Ian Carmody - Patient: it just is a hard to remember sometimes and <3:30:54 PM> Ananya - Doctor: here <3:30:57 PM> Ian Carmody - Patient: I mean these medications aren't that expensive but you know I gotta go to the pharmacy every every month and get them refilled and remember to take them during the day it's just kind of annoying <3:31:05 PM> Ananya - Doctor: sure <3:31:10 PM> Ananya - Doctor: what what <3:31:12 PM> Ananya - Doctor: to change do you think we could make to you know the number of medications you're taking but also be able to manage your conditions <3:31:21 PM> Ian Carmody - Patient: um <3:31:23 PM> Ananya - Doctor: yeah sometimes you know having a healthy diet and exercise can help with a lot of conditions that patients have and some of the ones you have to like your high blood pressure and your diabetes those can definitely be better managed if you eat well and exercise well so can you tell me a little bit about your exercise and diet currently <3:31:24 PM> Ian Carmody - Patient: I don't know if there's <3:31:28 PM> Ian Carmody - Patient: front medication to take or <3:31:31 PM> Ian Carmody - Patient: I guess you have talked to me before about maybe eating a little differently <3:31:51 PM> Ian Carmody - Patient: do <3:31:51 PM> Ian Carmody - Patient: mm-hmm yeah for exercise I don't really <3:32:06 PM> Ian Carmody - Patient: anything I usually get home from work <3:32:09 PM> Ian Carmody - Patient: um I work I just sit there at my desk and then I come home and I <3:32:14 PM> Ian Carmody - Patient: will usually <3:32:16 PM> Ian Carmody - Patient: and <3:32:16 PM> Ian Carmody - Patient: just sit and watch TV with my wife <3:32:19 PM> Ian Carmody - Patient: eat dinner we usually don't have that much energy after work so we might just order out get some like Chinese food or whatever is close to our house just kind of pop on the couch eat that and watch TV and then I just mostly get a lot of like frozen meals for work because <3:32:38 PM> Ian Carmody - Patient: um it's just <3:32:40 PM> Ian Carmody - Patient: to grab in the morning before work so I guess <3:32:41 PM> Ananya - Doctor: more <3:32:44 PM> Ananya - Doctor: yeah and um do you see any <3:32:44 PM> Ian Carmody - Patient: lot of salt in my diet and a lot of fat I don't really cook anything myself <3:32:55 PM> Ananya - Doctor: concerns with how much fat and <3:32:55 PM> Ananya - Doctor: like <3:32:58 PM> Ananya - Doctor: a Saul in your current diet <3:33:02 PM> Ian Carmody - Patient: yeah I mean I know the salt makes my blood pressure even higher and I know I'm on two blood pressure medications right now so <3:33:03 PM> Ananya - Doctor: sure I guess on a scale of one or 0 to 10 How likely are you to maybe try to incorporate more home cooked meals and instead of the frozen meals and takeout food <3:33:12 PM> Ian Carmody - Patient: guess that would make sense that that could be a way to maybe cut down to one medication and then for the fat it is kind of I feel like it's a cycle where <3:33:24 PM> Ian Carmody - Patient: I'm tired because I eat it <3:33:26 PM> Ian Carmody - Patient: but then I eat it because I'm tired so I just kind of just keep doing it because it seems easy <3:33:48 PM> Ian Carmody - Patient: um I think I would go with like a <3:33:48 PM> Ian Carmody - Patient: five <3:33:49 PM> Ananya - Doctor: okay five out of 10 is there any reason it's not a six out of 10 <3:33:53 PM> Ian Carmody - Patient: out of 10 <3:33:55 PM> Ian Carmody - Patient: hmm I think it's just hard for me to imagine doing that every single day <3:34:06 PM> Ian Carmody - Patient: and I'm not the best at cooking so it would just be a big change to do that every single day <3:34:07 PM> Ananya - Doctor: sure and what other concerns can you maybe see in the future if you continue to not exercise as much and not eat a very healthy diet <3:34:18 PM> Ian Carmody - Patient: mmm well I'm already pretty heavy and so I I've kind of noticed in the past few years my stomach has just been getting bigger and bigger so definitely that it makes it harder to get around the house too and I'm getting a little up there in age so my my knees are definitely hurting more with the increased weight and I have had the heart attack before where I got the stent so that could happen again <3:34:24 PM> Ananya - Doctor: yeah so I mean it sounds like there are a few different things that you are worried about if you continue your current behaviors so <3:35:01 PM> Ananya - Doctor: kind of going back to you know your medications and your frustration with them do you feel like it would be a good plan to maybe continue your medications but also work on diet and exercise <3:35:14 PM> Ian Carmody - Patient: yeah I think <3:35:18 PM> Ian Carmody - Patient: I think I could definitely be better about taking every day you know I skip on my water pill a lot just because I don't like having to pee every <3:35:18 PM> Ian Carmody - Patient: every <3:35:27 PM> Ian Carmody - Patient: might need to get up to go anywhere so that's important <3:35:32 PM> Ian Carmody - Patient: and if I could take less pills just by cooking at home that would be a great idea <3:35:33 PM> Ananya - Doctor: yeah and how do you think it would make you feel if you worked on you know diet exercise continuing to take your medications with some of your concerns about the future how do you think it would make you feel <3:35:42 PM> Ian Carmody - Patient: well yes yeah yeah I think it makes me feel a little bit more sure of myself and that maybe a less scared that I would have to <3:35:53 PM> Ananya - Doctor: yeah <3:36:00 PM> Ian Carmody - Patient: have another like surgery or something like that where I'm in the hospital <3:36:06 PM> Ananya -
