Patient: work man I just can't keep taking all of these I'm just I'm just over it <3:29:32 PM> katie - Doctor: um all of the medications you're on <3:29:41 PM> Sydne - Patient: I'm frustrated I just can't keep doing it <3:29:45 PM> Sydne - Patient: I'm I mean <3:29:48 PM> Sydne - Patient: what concerns me the most is that I don't know the name of this one I don't know the name of that one I don't even know what diseases I have for all I know I'm I'm a healthy 60 year old woman I am healthy as a 23 year old woman I'm <3:29:48 PM> Sydne - Patient: I'm <3:30:05 PM> Sydne - Patient: healthy and I don't even know what I need these for <3:30:09 PM> katie - Doctor: do you so we've been seeing each other for a little bit and kind of spent some time talking about <3:30:09 PM> Sydne - Patient: and then they told me diabetic I got you know diabetes <3:31:12 PM> Sydne - Patient: I take the foreman <3:31:14 PM> Sydne - Patient: I keep taking that form and I take it for all these years <3:31:17 PM> katie - Doctor: mm-hmm <3:31:18 PM> Sydne - Patient: and it's not doing anything now I have to take insulin <3:31:22 PM> Sydne - Patient: and then I and then I have a heart attack and then one day I have a heart attack and now I'm on all these other medications <3:31:30 PM> Sydne - Patient: and I'm just I can't keep taking all of these but <3:31:34 PM> Sydne - Patient: mean what I don't what's gonna happen if I just stop taking them not I feel fine you know <3:31:41 PM> Sydne - Patient: I don't think anything's going to happen <3:31:43 PM> katie - Doctor: yeah I think you know you have a lot brought up a lot of your concerns and I understand how it could be really frustrating to take have to take a lot of different medications in the morning so you talked about your blood pressure do you take your blood pressure at home <3:31:44 PM> Sydne - Patient: yeah I do <3:31:59 PM> katie - Doctor: and what numbers have you seen <3:32:02 PM> Sydne - Patient: um well I I take that medication I do because you said it's important so it's it's like 130 over 80 <3:32:09 PM> katie - Doctor: okay and that's great and not so how do you think your blood pressure would be if you weren't taking your medication <3:32:12 PM> Sydne - Patient: oh I remember that number that was like 160 over 90 that was a real bad one <3:32:19 PM> katie - Doctor: so yeah and that <3:32:30 PM> katie - Doctor: um so your blood pressure the medication would you say that the medication helps your blood pressure then <3:32:34 PM> Sydne - Patient: mmm yeah I guess so doc <3:32:36 PM> katie - Doctor: okay <3:32:40 PM> katie - Doctor: okay and so that's I'm really glad you take your blood pressure medication and keeping that down is really helpful as well <3:32:49 PM> katie - Doctor: I think you know you mentioned the diabetes and the heart disease and it can be harder to kind of see <3:32:52 PM> Sydne - Patient: mmm <3:32:57 PM> katie - Doctor: tangible results there um do you ever take your blood sugars at home <3:33:01 PM> Sydne - Patient: ah yeah you know and I'm on the insulin I have you know the little thing on my arm so yeah you know it takes my sugars <3:33:03 PM> katie - Doctor: and what sugars have you seen what numbers have you seen <3:33:13 PM> Sydne - Patient: oh I haven't been real good about that they've been around 200 to 250 <3:33:16 PM> katie - Doctor: okay <3:33:21 PM> katie - Doctor: have your sugars ever gotten really really high or really really low <3:33:23 PM> Sydne - Patient: oh <3:33:28 PM> katie - Doctor: okay <3:33:31 PM> Sydne - Patient: 100 not too bad you know I ate sweets I got a sweet tooth <3:33:35 PM> katie - Doctor: do you have any concerns about <3:33:36 PM> Sydne - Patient: fit <3:33:39 PM> katie - Doctor: if your sugars were to continue being high do you have any concerns about that <3:33:43 PM> Sydne - Patient: oh well you know I ended up in the hospital once I don't want to do that again <3:33:44 PM> katie - Doctor: what concerns would you have if your sugar is remained really high <3:33:53 PM> katie - Doctor: okay <3:33:54 PM> Sydne - Patient: what the high sugars <3:33:55 PM> katie - Doctor: yeah <3:33:55 PM> katie - Doctor: so <3:33:56 PM> Sydne - Patient: Hi Guess here hi I see where you're going with this doc I should probably keep taking my insulin huh <3:33:58 PM> katie - Doctor: hmm <3:34:04 PM> katie - Doctor: it would so it's again diabetes is a very <3:34:08 PM> katie - Doctor: it's a really hard it's a you know it's a very time-consuming <3:34:13 PM> katie - Doctor: disease to manage and so I do feel for you that it is it takes a lot of mental effort and you have to work on your insulin you have to work on your what you're eating but if maybe we're if we're able to implement some change a few small changes that could hardly help that could help you keep your sugars lower would you be <3:34:33 PM> katie - Doctor: willing to talk about that some more <3:34:36 PM> Sydne - Patient: yeah what kind of changes <3:34:38 PM> katie - Doctor: well <3:34:39 PM> Sydne - Patient: I don't know what changes no don't say more medications now I can't say more medications <3:34:41 PM> katie - Doctor: what do you <3:34:45 PM> katie - Doctor: so with diabetes what what is your diet look like every day what type of foods are you eating <3:34:49 PM> Sydne - Patient: I have a Ho-Ho for breakfast a ho ho for lunch and The Lean Cuisine for dinner <3:34:56 PM> katie - Doctor: okay the link Cuisine is great that's it's good to have some protein <3:34:59 PM> Sydne - Patient: mmm <3:35:05 PM> katie - Doctor: so with you having diabetes you know sometimes the more we have carbs and sugar which I know is the stuff that tastes tastes really good that can make our sugars run higher <3:35:06 PM> Sydne - Patient: yeah <3:35:19 PM> katie - Doctor: so things <3:35:21 PM> Sydne - Patient: yeah I like the sugars you're right <3:35:24 PM> Sydne - Patient: maybe I should cut out the ho-hos then shouldn't I <3:35:25 PM> katie -
